Fat contamination of pericardial suction blood and its influence on in vitro capillary-pore flow properties in patients undergoing routine coronary artery bypass grafting

Abstract: The pericardial suction blood plasma capillary flow function was highly impaired by liquid fat. Pericardial suction blood hemoglobin appears worth recovering after fat removal, despite profound hemolysis.

“…In accordance with our previous observations [25], PSB plasma was demonstrated to be severely deteriorated with respect to its capillary flow properties The PSB and MDB groups are not entirely comparable, but overall PSB plasma appeared to have a more deteriorated capillary flow resistance than MDB plasma. It is possible that wound fat lipids in PSB contributed to the observed difference.…”

“…The link between contaminating lipids from wound fat tissue and SCADs was proven in that cell saver washing of PSB reduced the embolic occurrence [1]. The biophysical consequences of lipid contamination on PSB plasma capillary flow resistance are profound [25]. Nevertheless, the multifactorial nature of diffuse brain damage is an intriguing challenge in the scientific search to identify side effects of PSB recycling.…”

“…The role of recycled PSB represents probably one crucial component. In addition to the deleterious effects related to fat embolism (2)(3)(4)9) and coagulation disorders (2,5,6,10,11) as previously addressed, other mechanisms of inflammatory responses associated with cardiotomy suction are scarcely investigated (11). We studied the release and concentration patterns of complement activation, white blood cells and proinflammatory cytokines during and after CPB.…”

“…Mechanical obstruction is of course one of them. It has been shown in vitro that mediastinal fat clogs filters and impairs circulation (12). This study shows a high uptake of lipid material in the kidneys, and it is therefore likely that these emboli cause a mechanical obstruction to blood flow.…”
