Favorable Outcome of Crescentic IgA Nephropathy Associated with Methicillin-ResistantStaphylococcus aureusInfection

Abstract: Dominant or codominant IgA deposits in the setting of proliferative glomerulonephritis usually indicate IgA nephropathy, Henoch-Schönlein purpura nephritis, or, sometimes, lupus nephritis. However, a new type of poststaphylococcal glomerulonephritis with predominantly IgA deposition has been increasingly reported. Herein, we report an unusual case of rapidly progressive glomerulonephritis following methicillin-resistant Staphylococcus aureus infection. Renal biopsy showed crescentic IgA nephropathy. The renal … Show more

“…The typical treatment for most cases of IgA-dominant IAGN was eradication of the infection. Treatment with antibiotics was performed in 57 patients, and 15 of these patients also received steroids/immunosuppressants [9,10,18,22,23,25,28,30,32,34]. Two patients were treated with steroids and/or immunosuppressants alone [22,31] (table 1).…”

“…There have been some reports of successful treatment of IgA-dominant IAGN with antibiotics only [5,6,20,23]. Steroids therapy may play a role in the treatment of resistant cases who do not respond to antibiotics alone and several case reports of successful treatment of resistant cases with steroids [25,30,32]. The roles of steroids have been controversial for their indications or complications including relapse of infection [10,23].…”

“…The role of steroids to treat this condition remains unclear 5 8. Immunosuppression with corticoids can be considered after the infection is controlled, if there is progression of the renal disease, as in our patient 14 15…”

“…Most of the causative pathogens of this disease are MRSAs, and favorable renal outcomes have been reported after timely administration of antibiotics and adequate immunosuppressant therapy. [ 4 , 5 ] Regarding the emerging importance of MRSA-related IgA nephropathy, we present a patient who had an MRSA infection without awareness of early sign of GN, resulting in a devastating renal outcome with crescent formation from IgA-dominant postinfectious GN.…”
