Features and prognosis of chemotherapy-treated Hodgkin's disease with initial bone marrow involvement

Abstract: The features and prognosis of Hodgkin's disease with bone marrow involvement were studied in a series of 53 patients. This form of the disease is characterized by the high incidence of clinical and biological signs reflecting disease activity, common cytopenia (which is rare in other forms), an increased incidence of the lymphocyte depletion histologic type, and extensive lymphoid involvement, often with splenomegaly. In bone marrow biopsy specimens, Sternberg-Reed cells are found in 80% of cases and fibrosis … Show more

“…It is well known that bone marrow involvement in Hodgkin disease occurs in older patients [6-81 with a higher percentage of lymphocyte depleted histologic subtype [6,8], is associated with a lower white blood cell count [6,7], platelet count [6,7], and hemoglobin concentration [6-81, advanced stage [4,5,8], and B symptoms [4-81. Our data support each of these points. None of these parameters are sufficiently sensitive to warrant a recommendation for a change in the AM Arbor staging procedure criteria.…”

“…Two of nine (22%) patients with lymphocyte-predominant histology also had marrow involvement, and neither had systemic symptoms. Bone marrow involvement is uncommon in this histologic type [13,20,21].…”

“…We also detected significant differences in pretreatment values of hemoglobin, leukocytes, and platelets in patients with a positive marrow biopsy. While some investigators noted similar findings [ 11,13,20,21], others have not [10,18,19]. It is noteworthy, however, that isolated cytopenias unrelated to marrow metastasis are not uncommon in Hodgkin's disease and may be secondary to diverse mechanisms, including hypersplenism and immune phenomena.…”
