Fibro-osseous lesion of maxilla. Report of two cases in a family with review of literature

Abstract: Fibrous dysplasia is a disturbance of bone metabolism that is classified as a benign fibro-osseous lesion. Fibrous connective tissue containing abnormal bone, replaces normal bone. The etiology of fibrous dysplasia is unknown. The radiographic appearance of the irregularly shaped trabeculae aids in the differential diagnosis. Occurring most commonly in the second decade of life, the lesions of fibrous dysplasia can be surgically recontoured for esthetic or functional purposes once they become dormant.

“…The other radiological manifestations are those resembling the surface of an orange (peau d'orange), a wispy arrangement (cotton wool), organization of the abnormal trabeculae into a swirling pattern similar to a fingerprint [11]. In our case, there was a well-defined radiographic margin which favoured a diagnosis of juvenile ossifying fibroma, unlike fibrous dysplasia which blends with surrounding bone [12]. …”

“…Fibrous dysplasia manifests as a painless swelling to the affected bones and produce aesthetically visible deformities. The deformity of the jaw results from a progressively slow growing painless swelling, but growth often slows or become arrested at a time coinciding with the onset of puberty. 6 The patient may experience a variety of symptoms, including headaches, loss of vision, proptosis, diplopia, loss of hearing, anosmia, nasal obstruction, epistaxis, epiphora and symptoms like sinusitiswhen the anatomical spaces and foramina are constricted because of encroachmentof the lesions. 19 Most of the patient present painless diffuse swelling in our study and some of patients develop a variety of symptoms, including headaches, nasal o-bstruction, epistaxis and symptoms like sinusitis.…”

“…The reactivation of the lesion can be seen due to hormonal changes, such as those seen in pregnancy. 8 When maxilla is affected it may involve zygomatic and sphenoid bone. Involvement of frontal, sphenoid, naso-ethmoid and maxillary bone may lead to nasal obstruction, sinus obstruction and sinusitis.…”
