Abstract:Malignant mixed tumor of the parotid is known to have odd sites for metastases. We describe the fine-needle aspiration cytology (FNAC) findings of pleural metastasis from a malignant mixed tumor misdiagnosed as a mesothelioma on cytology at the onset.A 47-year-old man presented to us with breathlessness and a massive pleural effusion with pleural-based nodules. He had been operated 2 years before for a pleomorphic adenoma of the parotid and had a healthy scar at that site. FNAC from the pleural lesions showed … Show more
“…In 1 case, a metastatic focus to the lung resembled a benign granuloma or hamartoma on CT imaging due to ossification of its osteosarcomatous element 7. In another case, a pleural metastasis was misdiagnosed as mesothelioma until tumor recurrence at the primary site in the parotid occurred 8…”
Review of the literature supports combined modality, surgery with postoperative radiation, as the standard of care for this malignancy, although the long-term prognosis for these patients is unclear.

“…The possibility of benign PA gave rise to malignant mixed tumor could not be excluded. Other two patients had the history of coexisting PA in the parotid gland and developed pleural and pulmonary metastasis, respectively [12,13]. A 10-yearold boy was simultaneously diagnosed with orbital metastasis and carcinosarcoma of the parotid gland [14].…”

“…Because there was only partial response and the right eye had no light perception, the enoculation was performed [14]. Other patient received pemetrexed and cisplatin based chemotherapy with partial response after the misdiagnosis of a mesothelioma [12]. The survival rate for this tumor has been reported as 0% at 5 years [10].…”
