Fine needle aspiration cytology of cylindroma of scalp: A case report

Abstract: Cylindromas are uncommon benign adnexal tumor. Multiple cylindromas are commonly genetically inherited and also known as "turban tumor." Cytology of dermal cylindroma is rarely discussed in previous literature. We have diagnosed a case of dermal cylindroma of scalp in an elderly female patient by fine needle aspiration cytology of the lesion and further confirmation was done by histopathology. Cytology consists of many clusters of small round to oval basaloid cells surrounded and attached with magenta coloured… Show more

“…When a tumour that morphologically resembles a cylindroma recurs, differentiating a malignant cylindroma from a benign cylindroma becomes crucial. Similarly, when the true nature of the initial tumour is not known, as was in our case, differentiating it from other skin adnexal tumours like eccrine spiradenoma, trichoepithelioma and other primary malignant or metastatic tumours like adenoid cystic carcinoma and basal cell carcinoma becomes important [ 3 , 5 , 8 ].…”

“…Despite known benefits of accurate preoperative diagnosis, scanty literature exists describing specific cytomorphological findings of individual adnexal tumours, especially cylindroma [ 3 - 8 ].…”

“…4 Although associated with trichoepithelioma and occasionally spiradenoma. 2,6 Cylindroma are conventionally classified as a neoplasm of apocrine differentiation, but the ultrastructural and immunohistochemical evidence of eccrine origin has been documented. 4 Massoumi et al stated that epithelial hair follicle origin and its exact pathology of differentiation (follicular vs apocrine vs sebaceous and even eccrine) may be dictated by the epithelial-mesenchymal signaling.…”

“…However, the location of the tumor in the subcutaneous plane with the absence of mitosis, mucin and nuclear atypia indicated a dermal origin of the tumor. 2,10 The other differential is chondroid syringoma which has a fibrillary chondromyxoid stroma with epithelial cells embedded. The epithelial cells are more of flat sheets with oval nuclei and fine chromatin.…”

“…70). Turban tumor is a name for multiple cylindromas on the scalp (Pal et al 2016). Only cylindromas that present as small dermal nodules are not completely hairless (Griffiths et al 2016).…”
