First Branchial Cleft Anomalies Involving the Tympanic Membrane and Middle Ear

Abstract: First branchial cleft anomalies may involve the tympanic membrane or middle ear. A complete otologic examination must be performed in any patient with a suspected first branchial cleft anomaly. A surgeon treating such a defect must be prepared to perform both the excision of the lesion and reconstructive otologic surgery.

“…The lesion can involve the tympanic membrane as an isolated cyst or pass through the membrane into the middle ear. It may also enter the middle ear directly through the tympanic bone, bypassing both the external canal and tympanic membrane 11 . In our series, there were no ossicular abnormalities or destructive process associated with middle ear involvement.…”

“…Middle ear and tympanic membrane. Tympanic membrane and middle ear involvement is unusual, and only occurs with type II branchial cleft anomalies 1,11,17 . There have been few case reports in the literature, and this paper presents the largest series with tympanic membrane or middle ear involvement.…”

“…A duplicated, squamous epithelium-filled, cystic mass surprisingly extended from the external auditory canal to the nasopharynx through the Eustachian tube. Descriptions of communication of the anomalous tract with the nasopharynx are extremely rare in the literature [17][18][19][20][21].…”

“…In this region, cysts and fistulas usually derive from the first branchial cleft. Due to extension to the external auditory meatus or, rarely the middle ear, anomalies of the first branchial cleft can also cause repeated otorrhea [2,3,8]. We report our experiences in managing an unusual sinus of the styloid process that was the cause of upper neck and ear infections in an adult patient.…”
