Fistula First Initiative: Historical Impact on Vascular Access Practice Patterns and Influence on Future Vascular Access Care

Abstract: The vascular access is the lifeline for the hemodialysis patient. In the United States, the Fistula First Breakthrough Initiative (FFBI) has been influential in improving use of arteriovenous fistulas (AVF) in prevalent hemodialysis patients. Currently, prevalent AVF rates are near the goal of 66% set forth by the original FFBI. However, central venous catheter (CVC) rates remain very high in the United States in patients initiating hemodialysis, nearly exceeding 80%. A new direction of the of the FFBI has foc… Show more

“…Hemodialysis (HD) is the most widely used dialysis therapy in the world, and vascular access represents an important risk factor for bacteremia, hospitalization, and mortality (Sesso et al, 2016;Niyyar, 2012;Lok and Mokrzycki, 2011;Al-Balas et al, 2017;Vascular Access 2006Work Group, 2006. In particular, catheter-related bloodstream infection (CR-BSI) is a severe complication that requires hospitalization, systemic antibiotic therapy, and removal, reinsertion or replacement of the catheter (The NIH, 2016;Lee, 2017;Sequeira et al, 2017). Various measures have been studied for the prevention of CR-BSI, one of which is the use of lock solutions (Katneni and Hedayati, 2007;Böhlke et al, 2015).…”

“…Solutions for locking prophylactic therapy can include antibiotic or non-antibiotic antimicrobial agents. The former are bactericidal and bacteriostatic agents (Lok and Mokrzycki, 2011;Lee, 2017). Bactericidal drugs cause death and disruption of the bacterial cell and include drugs that primarily act on the cell wall (e.g., β-lactams), cell membrane (e.g., daptomycin), or bacterial DNA (e.g., fluoroquinolones).…”

“…For patients who are treated with HD, the risks of major cardiovascular events, fatal and non-fatal infections and overall mortality are far greater with catheters than with AV fistula [3][4][5][6]. Although the AV fistula is cited as the preferred access for most patients, a majority of patients are treated with tunnelled dialysis catheters [7][8][9]. In response, recommendations from professional societies and governmental organisations, along with clinical guidelines from expert groups, have advocated strongly for changes in clinical practice that would favour greater utilization of the AV fistula over catheter use in order to improve patient outcomes [10][11][12].…”
