BackgroundTo investigate the short and medium term outcomes of bone defect reconstruction with bone cement prosthesis after clavicle malignancies resection.MethodsA total of 5 clavicular malignancy patients experienced bone cement prosthesis reconstruction after subtotal claviculectomy were enrolled the study from January 2005 to May 2012. Musculoskeletal Tumor Society score (MSTS), Visual Analogue scale (VAS) and American Shoulder and Elbow Surgeons shoulder outcome score (ASES) were adopted for assessment.ResultsThe mean follow-up period was 25.8 months. All patients were performed bone cement defect reconstruction after claviculectomy. In which, 3 cases showed disease-free and other 2 cases were alive with sickness. The average Musculoskeletal Tumor Society score was 85.40% ± 5.68%(77%-90%), Visual Analogue Scale was 1.40 ± 0.55 (1–2) and American Shoulder and Elbow Surgeons Shoulder Outcome Score was 92.40 ± 3.29(87–96).ConclusionsAdoption of clavicle bone cement prosthesis for bone defect reconstruction after tumor resection can maintain the contour of shoulder and reduce the complications ascribe to the claviculectomy. It is an effective and feasible therapeutic procedure in clinical setting.

Summary:The free vascularized fibula flap has been widely used for clavicle reconstruction. Limited evidence exists for the long-term outcome of clavicle reconstruction using the free vascularized fibula flap in adults. We report the functional and aesthetic outcome in a 52-year old man a decade after clavicle reconstruction using a free vascularized fibula flap in combination with a modified Richardson Hook Plate. At the 10-year follow-up, panoramic shoulder X-ray showed the modified Richardson Hook Plate had remained firmly in place with the fibula and the hook positioned beneath the acromion. Functionally, the patient presented with a constant shoulder score of 77, with a pain-free symmetrical full range of motion. In conclusion, reconstruction of lateral clavicle defect using free vascularized fibula flap in conjunction with modified Richardson Hook plate may provide patients with excellent long-term functional and aesthetic outcomes.

Ключица является очень редкой локализацией как первичного опухолевого процесса, так и метастатического поражения костей. В связи с этим в литературе встречается мало сообщений о хирургическом лечении этой патологии. В основном авторы описывают опыт выполнения тотальной либо субтотальной клавикулэктомии без замещения дефекта. Существуют единичные публикации о методах замещения дефектов при помощи аллотрансплантата ключицы, аутотрансплантата из ребра или малоберцовой кости, либо индивидуальных имплантатов. Мы представляем результат лечения метастатического поражения ключицы при семиноме яичка. пациенту была выполнена резекция новообразования с замещением пострезекционного дефекта конструкцией, состоящей из пластины с угловой стабильностью и костного цемента. Функциональный результат через 3 мес. по шкале MSTS составил 27 баллов, достигнут полный объем движений в левом плечевом суставе. предложенный метод может применяться у пациентов молодого возраста с первичным опухолевым процессом или метастатическим поражением ключицы, предъявляющих высокие требования к функциональному и косметическому результатам. Ключевые слова: метастатическое поражение ключицы, метастазирование семиномы, клавикулэктомия.
