From laboratory science to six emergency medical services systems: New understanding of the physiology of cardiopulmonary resuscitation increases survival rates after cardiac arrest

Abstract: Objective The purpose of this study is to: 1) describe a newly discovered mechanism of blood flow to the brain during CPR using the impedance threshold device (ITD) in a piglet model of cardiac arrest, and 2) describe the survival benefits in humans of applying all of the highly recommended changes in the 2005 Guidelines related to increasing circulation during CPR, including use of the ITD, from 6 EMS systems in the United States Design Animal studies prospective trial with each piglet serving as its own co… Show more

“…35,7 Delayed application of the ITD, failure to prevent airway leaks, and suboptimal performance of CPR can interfere with the hemodynamic improvements that are associated with ITD use. 1922 For these reasons, the ROC investigators implemented comprehensive training, retraining, electronic monitoring of the CPR process, and follow-up quality-assurance monitoring. The quality of the CPR provided was associated with outcomes in the sham-ITD group that were better than expected.…”

“…The intrathoracic vacuum that develops with the ITD helps to draw blood back to the heart and lower intracranial pressure during the recoil phase of CPR. 29 Although this device is attached to an airway, it is used during CPR to enhance circulation (Fig. 9).…”

“…1 Emergency Medical Services (EMS) treatment for OHCA significantly affects survival, 2, 3 and OHCA has been recognized as a priority condition for EMS research. 4 However, relevant patient outcomes in OHCA, such as neurologically intact survival, can often not be measured at the time of the EMS encounter.…”
