Functional Rhinoplasty

Abstract: The complete rhinoplasty surgeon must possess an understanding of functional nasal airway obstruction. An increasingly sophisticated grasp of the pathophysiology of fixed nasal obstruction has led surgeons to develop and refine surgical techniques aimed toward alleviating nasal valve insufficiency. This article reviews an assortment of techniques within nasal valve surgery, highlighting the underlying pathophysiology, anatomy, and technical considerations.

“…The nasal valves have been proposed to be a major regulator of nasal airflow, causing resistance and preventing airflow from exceeding the capacity to warm and humidify inspired air [2, 3]. The external nasal valve describes an area of the nasal vestibule bounded by the alar rim, nasal sill, and caudal septum [4, 5].…”

“…The NVA is formed by a combination of the internal nasal valve (INV), the external nasal valve (ENV), and the intervalve area (IVA) [6,8]. The INV is the most narrow point of the anterior nasal airway.…”

“…Several different types of grafts have been used such as lateral crural strut grafts and spreader grafts [1], sometimes in combination with alar batten grafts [8]. It is difficult to assess the effectiveness of alar batten graft placement alone.…”

“…Functional rhinoplasty procedures for INVI have evolved over the past decade. A wide range of techniques exists in which surgeons use alar batten and/or spreader grafts, butterfly grafts, lateral crural strut grafts, alar rim grafts [1], and lateral suspension sutures. Alar batten grafts were first shown by Toriumi et al [2] as an effective technique for correction of internal nasal valve collapse.…”
