Body image and quality of life in patients with and without body contouring surgery following bariatric surgery: a comparison of pre- and post-surgery groups

Background: Massive weight loss (MWL) following bariatric surgery frequently results in an excess of overstretched skin causing physical discomfort and negatively affecting quality of life, self-esteem, body image, and physical functioning.Methods: In this cross-sectional study 3 groups were compared: (1) patients prior to bariatric surgery (n = 79), (2) patients after bariatric surgery who had not undergone body contouring surgery (BCS) (n = 252), and (3) patients after bariatric surgery who underwent subsequent BCS (n = 62). All participants completed self-report questionnaires assessing body image (Multidimensional Body-Self Relations Questionnaire, MBSRQ), quality of life (IWQOL-Lite), symptoms of depression (PHQ-9), and anxiety (GAD-7).Results: Overall, 62 patients (19.2%) reported having undergone a total of 90 BCS procedures. The most common were abdominoplasties (88.7%), thigh lifts (24.2%), and breast lifts (16.1%). Post-bariatric surgery patients differed significantly in most variables from pre-bariatric surgery patients. Although there were fewer differences between patients with and without BCS, patients after BCS reported better appearance evaluation (AE), body area satisfaction (BAS), and physical functioning, even after controlling for excess weight loss and time since surgery. No differences were found for symptoms of depression and anxiety, and most other quality of life and body image domains.Discussion: Our results support the results of longitudinal studies demonstrating significant improvements in different aspects of body image, quality of life, and general psychopathology after bariatric surgery. Also, we found better AE and physical functioning in patients after BCS following bariatric surgery compared to patients with MWL after bariatric surgery who did not undergo BCS. Overall, there appears to be an effect of BCS on certain aspects of body image and quality of life but not on psychological aspects on the whole.

ResumenIntroducción y Objetivo. Los pacientes sometidos a cirugía bariátrica presentan exceso de piel como resultado de su pérdida masiva de peso. Las cirugías de contorno corporal que persiguen solucionar sus problemas funcionales y estéticos no están exentas de complicaciones.Nos proponemos identificar los factores asociados con complicaciones en pacientes sometidos a cirugía de contorno corporal después de cirugía bariátrica. Analizamos datos demográficos, comorbilidades y variables quirúrgicas en una muestra poblacional recogida entre los pacientes intervenidos en nuestro centro académico de Medellín, Colombia.Material y Método. Información retrospectiva y prospectiva de 627 procedimientos quirúrgicos de contorno corporal en 512 pacientes recolectada durante 4 años. Después de un análisis bivariado, seleccionamos las variables con un valor de p ≤ 0.25 para análisis multivariado mediante modelo de regresión logística apropiadamente estimado, probado y ajustado, a fin de identificar las variables independientes que pudieran explicar las complicaciones.Resultados. De los 512 pacientes del estudio, 67 procedimientos (10.7%) tuvieron complicaciones. La edad promedio fue de 41.22 ± 9.9 años. El peso promedio y el índice de masa corporal (IMC) prebariátrico fueron de 111.3 ± 19.6 kg y 46.1 ± 10.8 kg/m 2 respectivamente. El peso promedio e IMC postbariátrico fueron de 69.9 ± 11.7 kg y 27.2 ± 4.2 kg/m 2 respectivamente. Las variables en el análisis bivariado que se asociaron a complicaciones fueron: apnea del sueño (p<0.028), cirugía ambulatoria (p<0.017), transfusiones (p<0.013), peso prebariátrico (p=0.019) peso postbariátrico (p<0.001), tiempo quirúrgico (p<0.001), estancia hospitalaria (p<0.001), hemoglobina posquirúrgica (p<0.007) y tejido resecado (p<0.012). El análisis multivariado evidenció las siguientes variables independientes significativas: estancia hospitalaria (OR 1.7, IC 95%: 1.19-2.51, p<0.004), y peso postbariátrico (OR 14.72, IC 95%: 3.08-70.41, p<0.001). No hubo casos de embolismo pulmonar, admisión a Unidad de Cuidados Intensivos, ni muerte.Conclusiones. La prevalencia de complicaciones en nuestra población fue baja. Los factores independientes asociados con complicaciones en el análisis multivariado fueron la estancia hospitalaria y el peso postbariátrico.
