Gamma Knife surgery for skull base chordomas and chondrosarcomas

Abstract: As an adjuvant treatment after resection, GKS is a reasonable option for selected patients harboring skull base chordomas or chondrosarcomas with a residual tumor volume of less than 20 ml. Dose planning with a generous treatment volume to avoid marginal treatment failure should be made at a marginal dose of at least 15 Gy to achieve long-term tumor control.

“…The actuarial 5-year survival rates following proton radiation therapy, as reported by Hug et al [65], were 100% for patients with chondrosarcoma and 79% for chordoma. Radiosurgery has been also proposed as an adjunctive treatment option [66]; with mean marginal doses between 14-16 Gy, the actuarial local tumor control rates at 5 years range between 32 and 76% [67][68][69]. Tumor volume \20 ml has been associated with better outcome [69].…”

“…Additionally, treatment of clival chordomas was heterogeneous in this series owing to its retrospective nature. Furthermore, pathological specimens are essential for definitive diagnosis of skull base chordomas to differentiate these tumors from chondrosarcoma and other benign tumors that have a much better prognosis (16,17,21). However, our treatment results included a patient diagnosed by radiographyically.…”

“…In comparison, standard photon radiation for skull base tumors has been reported with overall survival rate of 39% at 5 years in a series of 39 patients from the Mayo Clinic [8]. Only 5-and 10-year progression-free survival of 42 and 32%, respectively, have been reported with adjuvant gamma knife radiosurgery when residual tumor volume is\20 cc [16]. The advantage Wu et al [130] Case series of gamma-knife radiosurgery (GKS) in the setting of skull base chordoma is the delivery of higher radiation dose to the tumor volume while minimizing the dose to critical structures.…”

“…The burden from this disease is considerable given the effect on neurological function and loss of individuals who are in their prime productive years. The reported 5-year survival rates for chordoma vary between 47 and 80% [1,8,[13][14][15][16][17]. Despite aggressive surgical resection, chordomas recur, with approximately 20% of postsurgical patients expected to show recurrence at 1 year [7].…”
