Gastric remnant cancer: an old problem with novel concerns

Abstract: Curative resection has to be the goal of surgical management in patients with gastric remnant cancer. Concerning clinician should be sceptical about a newly developing cancer in order to detect it in an early stage and enhance resectability.

“…Previous studies report an overall surgical complication rate for remnant gastric cancer of 19-47 %, with operationrelated mortality rates of 2-13 % [9][10][11][22][23][24]. In our study, the overall complication rate was relatively high at 46 %, which might be partially explained by the fact that our study applied the Clavien-Dindo classification system strictly, and thus includes minor problems that could be resolved with just simple supportive care.…”

“…In comparison with primary gastric cancers, RGC are commonly detected at advanced stages with extended lymph node metastases or the infiltration of adjacent organs. 3,5,7 In addition, the symptoms of RGC are similar to those of post-gastrectomy syndrome. 5 However, when RGC is resected curatively, there have been reported to be no significant differences between both groups in survival.…”

“…The main factor responsible for these changes is duodenogastric reflux of bile and pancreatic juice. 7,12,13 On the other hand, RGC after malignant disease presumably originates from some precancerous conditions which had already existed before the initial operation. These cancers are most likely metachronous lesions.…”

“…The incidence of RGC was 1-5% in previous reports (Firat et al 2009;Ojima et al 2010;Tanigawa et al 2010). Because no typical symptoms and signs were available at early stage, RGCs were often reported to be detected at advanced stages and had higher LN metastasis rate in comparing with conventional gastric cancer (Tanigawa et al 2010;Elliot Newman et al 1997).…”

“…Remnant gastric cancer (RGC), as a unique clinical entity with relatively lower frequency in gastric cancer series, has similar metastatic pattern and surgical principle with gastric cancer (Firat et al 2009). The incidence of RGC was 1-5% in previous reports (Firat et al 2009;Ojima et al 2010;Tanigawa et al 2010).…”
