Gastrointestinal basidiobolomycosis: Beware of the great masquerade a case report

Abstract: Highlights We describe a previously healthy young patient with gastrointestinal basidiobolomycosis misdiagnosed as colonic malignancy. This is the first reported case in an adult patient from Oman suggesting that the disease is possibly under-recognized. Acquaintance with this exceptionally rare infection is critical for successful clinical outcome. Azoles are effective anti-fungal therapy for gastrointestinal basidiobolomycosis.

“…The Patient’s general symptoms like abdominal pain, anorexia, constipation, and weight loss were similar to other reported cases but the patient had no bleeding although GI bleeding does not show a predilection for any particular age group and has been reported in patients aged 1.5 years to 80 years in most of the cases [ 12 14 ]. The colonoscopy in our case was mostly normal.…”

“…The stomach, small intestines, colon, and liver are among the major organs that can be affected [ 12 , 13 ]. Nonspecific signs and symptoms make diagnosis difficult [ 14 ]. A prompt and accurate diagnosis is critical, especially when there are signs of obstruction or sepsis.…”

“…Colonic involvement in gastrointestinal basidiobolomycosis is commonly misdiagnosed as malignant mass commonly in the cecal or ascending colon with local invasion and, at times, involvement of distant sites [ 8 , 16 ]. Disseminated gastrointestinal basidiobolomycosis is rare but potentially fatal [ 17 ].…”

“…The condition is most commonly diagnosed post-surgery and after the analysis of the resected tissues. So, the final diagnosis frequently comes late in the disease course [ 8 , 16 ]. As a result, gastrointestinal basidiobolomycosis should be kept in the differential diagnosis of inflammatory diseases of the gastrointestinal tract.…”

“…Detailed examination of the thrombus and a section of the small intestine of our patient showed thin-walled, broad and septate hyphae, and these features are compatible with zygomycete-like fungi ( Figure 3 ). Diagnosis of Basidiobolus infection down to the species level is typically accomplished by microbiological culture as gold standard [ 44 ]. From the thrombus specimen that was extracted from the common femoral artery, and from a urine sample, a mould was grown reminiscent of a Basidiobolus species.…”
