We describe a 74-year-old man with a colonic metastatic squamous cell carcinoma (SCC) from the lung. His chest X-ray revealed an abnormal shadow in the right upper lobe. Computed tomography (CT) of the chest demonstrated a large lung tumor in the right upper lobe obstructing the right upper bronchus. Bronchoscopy revealed an easy-bleeding tumor in the right upper bronchus that was diagnosed as poorly differentiated squamous cell lung carcinoma. He underwent colonoscopy because he had a positive fecal occult blood test. Colonoscopy revealed a large protruding lesion with central ulceration in the descending colon. Histological examination of the biopsy specimen obtained from the colonic lesion revealed SCC. The lesion was diagnosed as metastatic colonic SCC. He had no abdominal symptoms. He underwent chemotherapy with an infusion of cisplatin 130 mg i.v. day 1, and docetaxel hydrate 100 mg i.v. day 1, repeated every 4 wk, followed by 4 courses of chemotherapy. The primary lesion shrank by less than 10% and was judged to be "Partial Response" (PR) after 3 courses of treatment. The patient still lived 23 wk after the diagnosis of metastatic colonic SCC. Colonic metastasis of primary SCC of the lung is rare.

Introduction: Lung cancer remains a leading cause of cancer deaths worldwide, and an estimated 50% of cases are associated with metastasis at the time of diagnosis. We present the case of a 45-year-old female smoker with a history of right upper lobectomy with radiation for lung adenocarcinoma who, nine months later, presented with abdominal pain and was found to have an isolated metastatic lesion to the cecum causing intussusception. Case description: A 45-year-old woman presented with a two-day history of abdominal pain, melena, fever, and chills. A diagnostic workup revealed a mass in the cecum and a colocolic intussusception. The patient underwent right hemicolectomy and was discharged following a slow recovery. Microscopic examination of the lesion revealed an adenocarcinoma histologically identical to the primary lung tumor. Further workup failed to uncover any further evidence of disease. The patient continues to be well after 23 years of clinical follow-up. Discussion: Metastasis to the colon is a rare event but represents advanced disease and the prognosis is poor. Symptomatic involvement of the colon has only been reported in fourteen previous cases. Conclusions: Although uncommon, metastatic disease from lung to colon should be considered in patients with lung and large bowel masses diagnosed within a relatively short time course. More likely are synchronous primaries or colon metastasis to the lung, but an accurate diagnosis is critical. Lung metastasis to the bowel portends a poor prognosis, but isolated metastatic disease can be surgically resected for cure as demonstrated in our case.

AbstractThe gastrointestinal tract (GIT) is an unusual site for metastasis. The rate of GIT metastases detected clinically is very low because of unspecific symptoms and signs of GIT involvement, which include general weakness, tiredness, weight loss, unspecific abdominal pain, fatigue, and anemia. We report clinical, endoscopic, and pathological patterns of two patients (malignant melanoma and primary lung tumor) with metastatic lesions in the gastroduodenum. The first case is a 59-year-old man with unspecific symptoms as nausea, vomiting and abdominal pain. He underwent resection of skin melanoma on his back one year before. Upper gastrointestinal endoscopy revealed two melanotic polypoid masses with ulcerations at the tip, one in the stomach and one in the duodenal bulb. Endoscopic biopsy of these polypoid masses and immunohistochemical stains confirmed the diagnosis of metastatic malignant melanoma. The second case is a 73-year-old man with a two-day history of melena and unspecific abdominal pain. Three weeks before, the patient was operated on for the adenocarcinoma of the lung. Endoscopy of the upper gastrointestinal tract revealed irregular polypoid mass with ulcerations at the tip: three of the stomach mucosa, two in the duodenal bulb and more than ten hemorrhagic polypoid masses at the desendent duodenum. Biopsies of these lesions confirmed the diagnosis of metastatic lung adenocarcinoma. In patients with a history of malignant melanoma and lung cancer unspecific symptoms, like abdominal pain, anemia, and gastrointestinal bleeding gastroduodenal metastases should be suspected. The diagnosis requires careful endoscopic examinations of the mucosa for metastatic lesions and biopsy with special immunohistochemical stains.
