Gehaftes Auftreten der Immunthyreoiditis bei Patienten mit chronischer Polyarthritis

Abstract: The frequent occurrence of both rheumatoid arthritis and autoimmune thyroiditis was already investigated with in part many conflicting results. We investigated a number of 792 patients (383 of them suffering from rheumatoid arthritis and 409 with osteoarthritis). In all patients antithyroid peroxidase and antithyroglobulin antibodies were determined. Patients with rheumatoid arthritis showed a significantly higher occurrence of circulating thyroid antibodies than those with osteoarthritis (9.1% versus 3.7%, p … Show more

“…The prevalence of AITD among patients with RA in our study, approximately 10% at diagnosis, is in line with previous studies. 6,8,13 The criteria for AITD, however, differ between studies, which, apart from the timing of the measurement, may be one explanation for the wide range of previously reported estimates of prevalence. 14,21 The timing of risk development seen in our study with an…”

“…Lead-time bias would not explain the previously reported increased occurrence of thyroid antibodies in patients with RA. 7 , 8 , 12 , 13 , 14 Besides lead time, the observed peak in risk of AITD shortly prior to diagnosis of RA may also reflect a possible critical causal link between the 2 autoimmune conditions, with one (AITD) triggering the clinical onset of the other (RA) in genetically predisposed individuals. Such a mechanism may explain the higher incidence seen among seropositive patients compared with seronegative patients, for whom there should not be any difference in diagnostic intensity or lead-time bias.…”

“…The prevalence in RA cases ranged from 0.5% in Morocco [58] to 27% in Slovakia [71]. Within the studies analyzed, 10 studies were from Europe [23, 6371] with prevalence ranging from 1% in Germany [65] to 27% in Slovakia [71]. Four studies were from the North American region [5962] where prevalence ranged from 2.1% [61] to 9.8% [60].…”

“…[26] Some others have called this a manifestation of rheumatoid arthritis. [27] Among the most important or frequent rheumatic manifestations, there are a mild non-erosive variety of arthritis, polyarthralgia, myalgia, and sicca syndrome without a true Sjögren's syndrome. [28] In our patients, 16 (8.6%) had associated rheumatoid arthritis [Table 1].…”
