Genitourinary Tuberculosis After Renal Transplantation—A Report of Three Cases with a Good Clinical Outcome

Abstract: Tuberculosis (TB) continues to be a major opportunistic infection after renal transplantation especially in the tropical countries. The risk of TB in renal transplant recipients is reported to be 20-74 times higher than that in the general population. Genitourinary TB that occurs after renal transplantation in uncommon and appears to present differently than genitourinary TB in the nontransplant population. An increased risk of graft rejection and graft loss has been reported. We report three cases of genitour… Show more

“…A graft nephrectomy was performed; however acid-fast bacilli could not be demonstrated in the background of histiocytic granulomas. A recent report from India has described three patients with posttransplant tuberculosis of urinary tract in whom the diagnosis was confirmed by detection of acid-fast bacilli in urine and all the patients responded favorably to antitubercular treatment [6]. Another report from India has described a patient with posttransplant tuberculosis with a kidney biopsy showing epithelioid cell granulomas and necrosis.…”

“…Lungs are the most common site for tuberculosis in posttransplant setting [2,3]. Although tubercular involvement of the graft has been reported to occur in association with disseminated tuberculosis, reports of isolated tuberculosis of the renal allograft are infrequent in the literature [4][5][6][7]. In the majority of such cases the diagnosis has been made after graft nephrectomy [8][9][10].…”

“…[7778] TB occurs in an estimated 0.35-6.5% of organ transplant recipients,[79] significantly increasing their mortality (up to 17.86%). [71] The incidence reported from India is much higher, between 5.7% and 13.5%,[80] which is probably related to the higher incidence of TB.…”
