Giant left atrium associated with massive thrombus formation

Abstract: Giant left atrium is a condition characterized by huge enlargement of the left atrium with a diameter exceeding 65mm. It is most commonly associated with long standing rheumatic mitral valve disease. We present a 45-year-old female patient with rheumatic mitral stenosis associated with giant left atrium occupied by an 11 × 10 × 5 cm thrombus weighing 500 gms. The patient underwent successful mitral valve replacement and thrombectomy through an inverted T-shaped biatrial incision.

“…We herein report a case of a giant RA seen in an adult patient with chronic rheumatic valve disease. The pathophysiology of a giant RA is not fully understood, as it is not seen in all patients with chronic valvular dysfunction who develop pressure and volume overload [ 2 ]. Interestingly, almost all previously reported cases of giant atrial enlargement in adults were in patients with a prior history of rheumatic heart disease ( Table 2 ).…”

“…In the event of persistent or frequent recurrent PSVT, this process can eventually lead to apoptosis of myocardial cells and fibrosis, resulting in permanent structural changes of the atria . One of the frequent structural changes seen in patients with atrial cardiomyopathy is left atrial enlargement, which is known to provoke stasis of blood flow and increase the risk of thrombus formation . In addition, atrial fibrosis is associated with poor atrial contractility, which would further promote stagnation of the flow.…”

“…Giant left atrium (GLA) is a rare condition closely associated with rheumatic mitral valve disease, seen in only 3%–4% of patients with rheumatic valvular heart disease. 1 The atrial cut-off size for defining GLA differs among investigators; 1 , 2 however, an anterior-posterior dimension of 65 mm is commonly used in the literature. 2 4 Because of the location of the left atrium, as it increases in size, adjacent organs may be compressed, resulting in complications.…”
