Giant primary amyloidoma of the tonsil

Abstract: A case of giant amyloidoma in the left tonsil with extensive osseous metaplasia and a scanty and patchy monoclonal population of IgG Kappa plasma cells, is presented.Localized tumoral amyloidosis is a rare, benign tumour of the upper aerodigestive tract. Organ-limited amyloidosis has been shown to be confined to various systems but, since the lesion was first described, only four cases situated in the tonsils have been reported in the English literature, and none of these had either osseous metaplasia or a mon… Show more

“…From previous investigations, it appears that the tonsils and lymph nodes are mainly affected by AL amyloidosis and only occasionally by other forms, such as AA amyloidosis (15 versus two characterized cases) [2,8,20,25,33,34,49]. This may be due partly to the underlying disease which, as in our case, showed involvement of the lymphatic tissue and deposition of amyloid at the site of tumor progression.…”

“…Amyloid deposited in lymphatic tissue has been described (rarely) in the tonsil [2,6,7,10,14,26,31], is observed more frequently in lymph nodes and may be present in 6-23% of cases with generalized amyloidosis [8,20,25,33,34,36,38,49]. “…In the head and neck, amyloidosis may involve the nose 3 , paranasal sinuses 3,4 , nasopharynx, oropharynx, tonsils 5,6 , oral cavity, tongue, tracheobronchial tree and larynx. The larynx is the most usually involved site, and according to Mitrani 7 , in a descending order of prevalence, it involves: ventricular folds (55%), laryngeal ventricle (36%), subglottic space (36%), vocal folds (27%), aryepiglottic folds (23%) and anterior commissure (14%).…”
