AbstractThe aim of the study was to investigate the changes in gingival blood flow due to orthodontic forces. Eleven volunteers, with the maxillary canine in an ectopic position were tested. A Laser Doppler Flowmeter (LDF) with a gingival probe was used, registering both the blood flow and temperature of the gingivae. After baseline measurement, a fixed orthodontic appliance was bonded. Measurements were repeated monthly, after activation of the appliance. The study lasted 6 months. The baseline value was 338.7 ± 201.56 P.U. [Perfusion Unit (mean ± S.D.)] which decreased to 218.9 ± 74.83 P.U. (p < 0.05) after two months and the final value of 363.9 ± 194.86 P.U was not significantly different from that initially (p > 0.5). The results showed that application of a force of 75 g resulted in a decrease in gingival blood flow up to 50%, but this returned to previous values after a few months. The study supports this measurement technique as a useful tool for monitoring gingival blood flow in long-term studies as well.

Objective: To evaluate the effectiveness of diode laser gingivectomy as an adjunct to nonsurgical periodontal treatment in the management of periodontal health among patients receiving fixed orthodontic appliance therapy (FOAT). Materials and Methods: Thirty patients undergoing FOAT with gingival enlargement were block randomized into two treatment groups. The test group received diode laser gingivectomy (940-nm diode laser, ezlase, Biolase Technology Inc) as an adjunct to nonsurgical periodontal treatment. The control group received nonsurgical periodontal treatment only. For both groups, five periodontal parameters were assessed at baseline, 1 month, 3 months, and 6 months: Plaque Index, Gingival Index, bleeding on probing, probing pocket depth, and Gingival Overgrowth Index. Intra- and intergroup variations in the periodontal parameters were determined over time. Results: Both groups showed statistically significant improvements in periodontal health over the study period (P < .05). However, significant improvements in periodontal health were evident earlier among the test group subjects (P < .05). The magnitude of improvement in periodontal health compared to baseline was greater in the test group than in the control group for Gingival Overgrowth Index at 1 month (P < .001) and 3 months (P < .05), Gingival Index at 3 months (P < .05) and 6 months (P < .05), and probing pocket depth at 1 month (P < .05). Conclusions: Nonsurgical periodontal management with or without the adjunct use of lasers can be effective in the management of gingival health problems among patients receiving FOAT. The adjunctive use of lasers can produce an earlier and greater improvement in gingival health.

La erupción pasiva alterada (epa) es una condición periodontal que puede alterar significativamente el logro de los objetivos estéticos del tratamiento de ortodoncia. Aunque esta alteración es conocida por los ortodoncistas, la mayoría de ellos no reconocen la diferencia en cuanto a su clasificación y manejo. Por tanto, la intención de este artículo es describir los lineamientos para realizar un diagnóstico certero, explicar su tratamiento y establecer un protocolo de manejo para la epa, que indique en qué momento remitir para que se realicen los procedimientos plásticos periodontales, basado en los hallazgos clínicos y teniendo en cuenta la edad como factor crítico para definir el procedimiento.Palabras clave: edad, erupción pasiva alterada, estética gingival, ortodoncia. Ortodontia and Passive EruptionAbstract. Altered Passive Eruption (ape) is a condition that can significantly affect achievement of the aesthetic goals of orthodontic treatment. Although this condition is known to orthodontists, most do not recognize the difference in its classification and management. The aim of this article is therefore to describe the guidelines for accurate diagnosis, explain the treatment and establish a management protocol for ape that would indicate when such patients are to be referred for plastic periodontal procedures, based on clinical findings and considering age as a critical factor when determining the procedure.
