Objective We performed a retrospective study to compare the accuracy of preoperative planning using three-dimensional AI-HIP software and traditional two-dimensional manual templating to predict the size and position of prostheses. The purpose of this study was to evaluate the accuracy of AI-HIP in preoperative planning for primary total hip arthroplasty. Methods In total, 316 hips treated from April 2019 to June 2020 were retrospectively reviewed. A typical preoperative planning process for patients was implemented to compare the accuracy of the two preoperative planning methods with respect to prosthetic size and position. Intraclass correlation coefficients (ICCs) were used to evaluate the homogeneity between the actual prosthetic size and position and the preoperative planning method. Results When AI-HIP software and manual templating were used for preoperative planning, the stem agreement was 87.7% and 58.9%, respectively, and the cup agreement was 94.0% and 65.2%, respectively. The results showed that when AI-HIP software was used, an extremely high level of consistency (ICC > 0.95) was achieved for the femoral stem size, cup size, and femoral osteotomy level (ICC = 0.972, 0.962, and 0.961, respectively). Conclusion AI-HIP software showed excellent reliability for predicting the component size and implant position in primary total hip arthroplasty.

Background Although the high offset Tri-Lock bone preservation stem (BPS) was used widely, few studies explored the clinical and radiological results. The purpose of this study was to determine the clinical and radiological results of high offset Tri-Lock BPS in unilateral primary total hip arthroplasty (THA) at a minimum follow-up of 3 years. Methods 55 patients who underwent cementless THA with high offset Tri-lock BPS from 2017 to 2018 were followed for a minimum follow-up of 3 years. Patients were assessed clinically for complications, Harris hip score (HHS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and Oxford Hip Score (OHS). Femoral offset (FO), acetabular offset (AO), hip offset (HO), HO difference, and leg length discrepancy (LLD) were measured on the anteroposterior (AP) pelvic radiograph. Standard pelvic AP and lateral radiographs were used to evaluate for evidence of bone ingrowth, stem subsidence, stem alignment, radiolucent line around the stem, osteolysis, loosening, ectopic ossification, and femoral stress shielding. Results No patients reported complications during hospitalization nor the follow-up period. At a mean follow-up of 42.5 months, the mean HHS, WOMAC, and OHS scores showed a significant improvement from preoperative to the latest follow-up. No patients reported thigh pain. No revision nor sign of radiographic loosening had been detected. The high offset Tri-Lock BPS significantly improved the FO and HO postoperatively. HO difference and LLD were balanced postoperatively. No sign of stem subsidence, radiolucent line, osteolysis, loosening, ectopic ossification, nor severe stress shielding (more than grade 3–4) were observed at the latest follow-up. Conclusion The high offset Tri-Lock BPS demonstrated excellent clinical and radiographic outcomes at a minimum follow-up of 3 years. HO difference and LLD between legs decreased significantly and achieved balance postoperatively. Long-term follow-up is required for a definitive conclusion.

A Retrospective Study of 98 Elderly Patients with High-Risk Lateral Femoral Wall Intertrochanteric Hip Fractures to Compare Outcomes Following Surgery with Proximal Femoral Nail Antirotation (PFNA) Versus Dynamic Hip Screw (DHS)

Background The aim of this study was to evaluate the efficacy of dynamic hip screw (DHS) and femoral nail antirotation (PFNA) in the treatment of lateral-wall high-risk type of intertrochanteric fractures. Material/Methods A total of 98 patients with high-risk intertrochanteric fractures of the lateral wall who underwent surgery from January 2019 to December 2020 were selected as the study subjects. Of these, 52 were treated surgically with PFNA (mean age 73.45±5.95 years) and 46 with DHS (71.37±6.22 years). We followed up these patients and compared the occurrence of the 2 surgical methods in terms of perioperative period, complication rate, and functional recovery. Results In terms of lateral wall fracture, there were 2 cases of PFNA and 10 cases of DHS, and the difference between groups was statistically significant ( P <0.05). Operative time (mean 54.94±7.29 vs 61.17±6.45) and intraoperative blood loss (72.80±9.18 vs 96.12±8.22) was significantly lower in the PFNA group compared to the DHS group (all P <0.05). Efficacy in the PFNA group was significantly better than in the DHS group. The HHS at follow-up was significantly higher in the PFNA group (mean 80.73±9.20 vs 64.19±8.12) than in the DHS group ( P <0.001). The VAS score at follow-up was significantly lower in the PFNA group (1.78±0.34 vs 2.65±0.23) than in the DHS group ( P <0.001). Conclusions PFNA is more effective than DHS in the treatment of high-risk lateral wall fractures in the elderly, with the advantages of lower incidence of complications and better recovery of hip joint function. PFNA warrants clinical application.
