Haematuria in patients with haemophilia and its influence on renal function and proteinuria

Abstract: To investigate renal function in a group of patients with a history of haemophilia and haematuria. We reviewed 32 medical records of the patients with haemophilia and gross haematuria identified through a computerized haemophilia registry, from January 1993 to December 2004. In all patients but three (two refused to participate and one died) clinical and laboratory tests were performed by the nephrologist. One patient had chronic renal failure because of diabetic nephropathy. In two patients reduced renal func… Show more

“…Van den Putte found a trend ( P = 0.05), but the information is based on a single question reported by patients and the paper does not clarify whether there was a correlation between renal functioning and haematuria episodes . It is known that other causes for haematuria apart from renal bleeds or renal disorders exist in the urinary tract or elsewhere . Even though the effect of haematuria on hypertension is possible, it might be small and it could also be that higher blood pressure triggers more haematuria or that only a part of macroscopic haematuria episodes have their root in kidney disease or kidney bleeding.…”

“…In a survey of the renal status in 26 people with hemophilia (age range, 17‐82 years), Beck and Evans reported a history of macroscopic hematuria in 69% of the patients. Later studies confirmed the high prevalence of hematuria among people with hemophilia . Although usually considered a benign condition, studies have found macroscopic hematuria associated with a reduction in renal function and an increased risk of hypertension, comorbid conditions that may complicate treatment in aging people with hemophilia.…”

“…In fact, cases of acute kidney injury secondary to tubular obstruction or cortical necrosis following renal bleeding treated with antifibrinolytic agents have been reported (25,26), and previous studies reported a reduction in kidney function in patients with haematuria (27). However, more recent data do not confirm these early findings, and now the role of kidney bleeding in renal disease pathogenesis remains to be clarified (28).…”
