Helical tomotherapy for total lymphoid irradiation

Abstract: Total lymphoid irradiation is employed in the preparative regimens for allogeneic bone marrow and solid organ transplantation, solid organ transplant rejection, and chronic graft-versus-host disease. Linear accelerator-based radiotherapy, typically involving opposed anteroposterior and posteroanterior beams, has been commonly used; however, extended source-to-skin patient setup and/or field matching are required, and all organs within the beam coverage receive the entire prescribed dose. Megavoltage helical to… Show more

“…McCutchen et al reported a single institution's experience of TLI performed by helical tomotherapy in an adult patient with late-onset cardiac transplant rejection (22). The prescribed dose was 8 Gy to 95% of PTV, in 10 twiceweekly fractions.…”

“…However, in this setting, more sophisticated RT techniques such as IMRT, IGRT, and especially helical-slit IMRT (HT) and cone-beam IMRT (RapidArc and VMAT) [ 30 ] might become appropriate alternatives for either definitive or palliative treatment for suitable patients based on compatible pulmonary toxicity criteria. Helical tomotherapy is a promising method, and achieves a better dose conformity in several tumor sites including MPM [ 31 - 33 ]. In their recent study, Sterzing et al compared step-and-shoot IMRT with HT [ 33 ].…”

“…Moreover, TOMO might provide safer and more accurate RT profile for selected HL patients with bulky residual disease. Last but not least, according to preliminary results TOMO might be administered for total lymphoid irradiation as the preparation for allogeneic bone marrow transplantation or as an alternative therapy for chronic graft-versus-host disease [ 50 ].…”
