Hemodynamic Function at Rest, During Acute Stress, and in the Field

Abstract: Abstract-Left ventricular hypertrophy is an independent predictor of cardiovascular morbidity and mortality. However, predictors of cardiac structure and function in youth are not completely understood. On 2 occasions (2.3 years apart), we examined 146 youth aged initially 10 to 19 years (mean age, 14.2Ϯ1.8 years). On the initial visit, hemodynamic function was assessed at rest, during laboratory stress (ie, orthostasis, car-driving simulation, video game, and forehead cold), and in the field (ie, ambulatory b… Show more

“…Left ventricular posterior wall in diastole, interventricular septum in diastole, and left ventricular internal diameter in diastole were measured according to the American Society of Echocardiography convention. 20 LVM was calculated using the necropsy-validated formula of Devereux et al and normalized to BSA to obtain LVMI, 21, 22 Intra- and interrater coefficients of variation for all cardiac structures assessed were 10%. 23 …”

“…Cardiovascular outcomes as indices of sympathetic activation to the heart (Kapuku et al, 1999) included systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR). Heart rate variability (HRV) expressed as the root mean square of successive N‐N differences (r‐MSSD, ms) was used as a measure of parasympathetic influence on the heart.…”

“…Family history of essential hypertension (EH) was defined as the occurrence of EH (i.e., SBP 140 mm Hg and/or DBP 90 mm Hg or antihypertensive medication) in 1 or both biological parents and a family history of premature myocardial infarction (MI) which was defined as the occurrence of MI in either biological parent or any biological grandparent before 55 years of age. Diagnosis of EH and of premature MI was verified by the subject’s physician or medical records 26, 27 . Inclusion criteria were: 1) African American or European American; 2) Healthy with no history of congenital heart defect or any chronic illness or health problem requiring pharmacological treatment.…”

“…At minute 20, 5-ml blood samples were drawn. Following the baseline evaluation, the subject engaged in two laboratory stressors (viz., video game and forehead cold) using standardized protocols 26, 27, 29 . The 10-minute video game stressor "Break Out" (Atari Inc, USA) was presented under a monetary incentive challenge.…”
