Hemostatic Balance in Severe Trauma

Abstract: Acute coagulopathy is prevalent in adult and pediatric trauma patients and is associated with increased morbidity and mortality. While reasonable hypotheses have been created to explain the underlying perturbations of adult trauma coagulopathy (i.e., tissue factor-related increase in thrombin generation, protein C activation, hypoperfusion, and hyperfibrinolysis), only a small number of studies have been performed to prove whether these mechanisms can likewise be detected in pediatric trauma patients. In addit… Show more

“…Increased activated PC activity is implicated in the pathogenesis of trauma‐induced coagulopathy and associated hemorrhage. 56 , 57 The bleeding tendencies are associated with activated PC‐induced inhibition of factors V, VIII, and plasminogen activator inhibitor. No dogs in our study had bleeding tendencies, and reports of hemorrhage in patients with GBM are rare.…”

“…So kann im Falle einer isolierten Erniedrigung des Quick-Werts die Gabe von aktiviertem Prothrombinkomplex (PPSB) sinnvoll sein. Ferner können Fibrinogen-Präparate bei schwersten Blutungen und deutlichem Fibrinogenmangel indiziert sein [22,23]. Auch die Gabe von aktiviertem rekombinantem Faktor VII ist in der Literatur beschrieben [24,25].…”

“…All patients in the database, either with or without TXA in the matched pairs table, had an injury severity score (ISS) above 25 [ 1 ]. Life-threatening coagulopathy is present in almost all patients with an ISS > 25 with hypotension, hypothermia, and acidosis [ 2 ]. Severe hypofibrinogenemia (< 100 mg/dl) is also a frequent finding in trauma with an ISS score above 25 [ 2 ].…”

Severe hypofibrinogenemia (< 100 mg/dl) is also a frequent finding in trauma with an ISS score above 25 [ 2 ]. In patients with disrupted coagulation in severe trauma with an ISS above 25, PPSB and factors VII are crucial [ 3 ].…”
