Hepatic sarcoidosis associated with chronic hepatitis C

Abstract: A 57-year-old woman with sarcoidosis was referred because of the appearance of multiple small low-attenuation areas in the liver on computed tomography (CT). A liver biopsy specimen showed chronic active hepatitis accompanied by sarcoid granulomas. The patient received prednisolone and, later, interferon-alpha. CT at 5 months of prednisolone treatment showed disappearance of the hepatic low-attenuation nodules. During long-term follow-up, however, these nodules reappeared on CT and liver cirrhosis finally deve… Show more

“…IFN‐α is the mainstay treatment for chronic HCV infection since its approval by the FDA in 1991, and that is probably why most cases of IIS have been reported in that population. Similar to other species of viruses and bacteria implicated in the aetiology of sarcoidosis (101), some reports have suggested a potential role for the HCV itself in the development of sarcoidosis (110–112). Chronic HCV infection is associated with immune dysregulation phenomena (such as essential mixed cryoglobulinaemia, autoimmune thyroid disease, sialoadenitis, diabetes mellitus and myasthenia gravis) (113–118) and induces chronic liver damage through a Th‐1 immune type of response (119).…”

“…Cases of HCV‐related sarcoidosis have been reported in treatment‐naive patients, 23–25 in those receiving antiviral therapy for HCV 7–13 and in those who had reactivation of previously known sarcoidosis while on antiviral therapy for HCV 14,26,27 . Overall, symptoms of sarcoidosis can occur any time after commencement of antiviral therapy and resolution of symptoms has been widely reported after cessation of antiviral treatment 9–13 .…”
