BackgroundThe immigrant population has increased greatly in Spain in recent years to the point where immigrants made up 12% of the infant population in 2008. There is little information available on the profile of this group with regard to prescription drug utilization in universal public health care systems such as that operating in Spain. This work studies the overall and specific differences in prescription drug utilization between the immigrant and Spanish population.MethodsUse was made of the Aragonese Health Service databases for 2006. The studied population comprises 159,908 children aged 0-14 years, 13.6% of whom are foreign nationals. Different utilization variables were calculated for each group. Prescription-drug consumption is measured in Defined Daily Doses (DDD) and DDD/1000 persons/day/(DID).ResultsA total of 833,223 prescriptions were studied. Utilization is lower for immigrant children than in Spanish children for both DID (66.27 v. 113.67) and average annual expense (€21.55 v. €41.14). Immigrant children consume fewer prescription drugs than Spanish children in all of the therapy groups, with the most prescribed (in DID) being: respiratory system, anti-infectives for systemic use, nervous system, sensory organs. Significant differences were observed in relation to the type of drugs and the geographical background of immigrants.ConclusionPrescription drug utilization is much greater in Spanish children than in immigrant children, particularly with reference to bronchodilators (montelukast and terbutaline) and attention-disorder hyperactivity drugs such as methylphenidate. There are important differences regarding drug type and depending on immigrants' geographical backgrounds that suggest there are social, cultural and access factors underlying these disparities.

Rev Esp Salud Pública 2003; 77: 713-723 N.º 6 -Noviembre-Diciembre 2003 Resultados: Recuperamos 8.444 ingresos que identificaron 1.577 ingresos de inmigrantes. Se evidenció una proporción similar de inmigrantes registrados con documentación reglada (841, 51,16%) e inmigrantes supuestamente no regularizados (803, 48,84%). Predominaron las mujeres (68,3%) frente a los varones (31,7%) y una media de edad inferior a los 30 años. El diagnóstico de alta más frecuente fue «complicaciones del embarazo, parto y puerperio» con 37,7% del total. Los «traumatismos y envenenamientos», «enfermedades del aparato digestivo» y «enfermedades del aparato respiratorio» representaron el 12,9%, el 7,8% y el 5,4%, respectivamente. Las «enfermedades infecciosas y parasitarias» supusieron el 4,4%.Conclusiones: La proporción de enfermedades infecciosas e infectocontagiosas observadas no contribuyen a alentar las opiniones difundidas sobre importación o reemergencia de enfermedades desde los colectivos de inmigrantes. La inmigración masiva acaecida requiere múltiples adaptaciones del Sistema de Salud que permitan establecer el perfil de salud de este colectivo, basado en estimaciones poblacionales.Palabras clave: Inmigrante. For theoretical reasons, the question has been raised as to whether this population may possibly be responsible for a rise in the incidence and/or transmission of infectious diseases. However, very few studies have been conducted regarding the diseases among this group which would afford the possibility of ascertaining the importance of this statement. This study is aimed at quantifying the hospital care provided to this population in the city of Valencia.Methods: The hospital release records were obtained from the Admissions Departments of the public hospitals in the city of Valencia. A oneyear retrospective observational study was conducted throughout the period.Results: We retrieved 8,444 hospital admissions identifying 1,577 hospital admissions of immigrants. Similar percentages of immigrants recorded with their documentation in proper order (841; 51.16%) and immigrants supposedly undocumented (803; 48.84%) were found. Females (68.3%) were predominant over males (31.7%), and a mean age of under 30 years of age. D i s e ñ o : estudio observacional con grupo control. Material y métodos: muestras de 147 hijos de inmigrantes (HI) y 147 hijos de la población autóctona (HA) apareados por edad y sexo, de los 4 C e n t ros de Salud de Toledo, recogiéndose de las historias clínicas datos demográficos, inicio y duración de lactancia materna, asistencia a consultas p rogramadas, situación calendario vacunal, número, motivos de consulta y pruebas complementarias solicitadas en un año (junio 2000-junio 2001). R e s u l t a d o s : el número medio de consultas/año fue similar en ambos grupos, 2,12 (DE 2,43) y 2,56 (DE 2,96) en HI y HA respectivamente. El 37,41% de HI faltó en alguna ocasión a consulta pro g r amada, frente al 17% de HA (p<0,05). El motivo de consulta más frecuente en ambos grupos fue infección respiratoria de vías altas (63,37% en HI v s 73,79% en HA). Las gastroenteritis agudas y las otitis fueron menos frecuentes en HI que en HA (9,89 v s 20,39%; 10,88 v s 23,3% re s p e c t i v a m e n t e , p<0,05), lo contrario que los dolores abdominales inespecíficos (9,9% en HI v s 1,94% en HA, p<0,05), el resto de motivos de consulta fue similar en ambos grupos. Se solicitaron 39 pruebas complementarias en HI y 29 en HA, destacando el ABSTRACT Objectives: to study the use of pediatric Primary Health Care consultations by inmigrants childre n (IC) and the morbility of this group, comparing to native children (NC). Design: observational study with control group. Material and methods: two samples of 147 IC and 147 NC matched by sex and age, from the 4 P r i m a ry Health Care Centres in Toledo. From the histories were collected dates about demography, beginning and lasting of breast feeding, attendance to programmed visits, vaccinal situation, demanding´s number and causes, and laboratory tests re q u i red along one year (June 2000-June 2 0 0 1). Results: the mean number of demands by year was similar in both groups IC and NC, 2.12 (SD 2.43) and 2.56 (SD 2.96) re s p e c t i v e l y. 37.21% of IC failed, at least once, to a programmed visit, comparing to 17% of NC (p< 0.05). The most f requent demanding cause was upper re s p i r a t o ry tract infection in both groups (63.37% at IC v s 73.79% at NC). Acute gastroenteritis and otitis w e re less frequent among IC than NC (9.98 v s 20.39%; 10.88 v s 23.3% re s p e c t i v e l y, p<0.05). The opposite happened with inespecific abdominal pain (9.9 at IC v s 1.94% at NC, p<0.05). The rest of demanding causes were similar in both g roups. Thirty-nine laboratory tests were asked for IC group and 29 for NC, being re m a r k a b l e Mantoux (four times for IC and none for NC).
