Hidradenitis Suppurativa Associated with Spondyloarthritis — Results from a Multicenter National Prospective Study

Abstract: SpA may occur in patients with HS, with the prevalence in this group exceeding that in the general population. The very short time between skin and joint symptom onset in some cases suggests common pathogenic mechanisms underlying HS and SpA.

“…It is also associated with other inflammatory syndromes with skin, bone and joint involvement: Synovitisacne-pustulosis-hyperostosis-osteitis (SAPHO), PASH syndrome (acne conglobata, pyoderma gangrenosum and HS), PASS syndrome (pyoderma gangrenosum, acne conglobata, HS and axial spondyloarthritis) and PAPASH syndrome (pyoderma gangrenosum, acne, psoriasis, arthritis and suppurative hidradenitis) [13,14]. Prevalence of inflammatory arthritis is increased in severe HS, with a predominance of spondyloarthritis (SA) [12,15].…”

“…HS was described to be associated with inflammatory syndromes such as pyoderma gangrenosum, Crohn's disease and Behç et's disease [13,14]. “…Recently, these facts seem to be confirmed by Richette et al who showed a prevalence of spondylo-arthritis of 3.7% in patients with HS, higher than in general population, and concluded that the very short time between skin and joint symptom onset in some cases suggests common pathogenic mechanisms underlying HS and spondylo-arthritis [20]. Interestingly, our two first cases had mainly peripherical manifestations, only the third had axial manifestations, this fact may highlight the relationship between the mechanism and the clinical presentation: the peripherical manifestations seem to be linked to pharmacological side effect.…”

“…Recently the first prospective study investigating the presence of SpA in patients with HS has found an elevated overall rate of 3.7% [31] . Within the screened 640 patients with HS, 184 (29%) had musculoskeletal symptoms, considerably fewer than in the present study.…”
