High-Concentration Nitrous Oxide for Procedural Sedation in Children: Adverse Events and Depth of Sedation

Abstract: In this largest prospective emergency department series, high-concentration continuous-flow nitrous oxide (70%) was found to be a safe agent for procedural sedation and analgesia when embedded in a comprehensive sedation program. Nitrous oxide also seems safe in children aged 1 to 3 years.

“…We did not perform any biopsy under general anesthesia. In different studies, it is noted that the majority of junior doctors, who are now trained in pediatric life, feel that short sedation is feasible and so the biopsies are easier to organize [12].…”

“…One area of attention and contraindication is the patient with pneumothorax or pulmonary blebs (due to the risk of gas accumulation) [153]. Vomiting occurs uncommonly (about 5% rate) even in "high-dose" (70 : 30) N 2 O administration [154]; nausea without vomiting occurred only once in a recent prospective trial of 50 : 50 N 2 O use in the ED [155]. The risk of nausea/vomiting appears to be increased with use of higher concentrations of N 2 O or with combination therapy of N 2 O and an opioid such as fentanyl [156].…”

“…Regarding the depth of sedation and incidence of adverse effects associated with inhalation of nitrous oxide in children 36 (B), it is known that high concentrations of the gas (70%) result in mild to moderate sedation with low occurrence of adverse effects, and that they safe for sedation and analgesia even in children under three years of age 36 (B). The occurrence of adverse effects -mostly vomiting -does not differ between the administration of nitrous oxide at 50% or 70%.…”
