High diagnostic performance of ELISA detection of antibodies to citrullinated antigens in rheumatoid arthritis

Abstract: ELISA detection of antibodies to citrullinated antigens, especially a second generation anti-CCP, showed higher discriminative ability than other assays, including RF, and would be useful to aid the diagnosis of RA in clinical practice.

“…8,14,15 An assay of CCP antibody of second generation was later established and the sensitivity and specificity of the assay were 66%-88% and 89%-98%, respectively. [16][17][18][19][20] The sensitivity and specificity of the anti-citrullinated type II collagen antibody assay were approximately 79% and 95%, respectively. Thus, the anti-citrullinated type II collagen antibody assay may become one of the significant tools for diagnosis of RA, although the sensitivity of the assay is less than that of the CCP antibody assay of second generation.…”

“…To date, a number of reports have demonstrated the clinical significance of autoantibodies to citrullinated filaggrin and CCP in the diagnosis of RA as summarized in Table 1 (3,5,6,(11)(12)(13)(14). Although the specificity of antifilaggrin/CCP antibodies in RA is more than 90% in almost all reports, the prevalence (sensitivity) of the same antibodies ranges from 33% to 87.2%.…”

“…It has relatively low specifi city due to being sometimes detected in patients with other autoimmune diseases and unaffected healthy people. In contrast, anti-CCP antibodies, which have higher specifi city than RF and comparable sensitivity with that of RF (De Rycke et al 2004), are good serologic markers for RA (Suzuki et al 2003;De Rycke et al 2004). Our study confi rms that anti-CCP antibody is more predictive than RF in RA patients.…”

“…Anti-CCP antibodies have been proved to be an effi cient diagnostic marker for RA, as they can be detected with high disease specifi city in more than 80% of serum samples taken from RA patients (Suzuki et al 2003). Moreover, anti-CCP antibodies are now considered as important markers for the diagnosis of RA and as probable prognostic markers for the development of erosive arthritis (van der Helm-van Mil et al 2005;Syversen et al 2007).…”
