High Preoperative Plasma Neopterin Predicts Delirium After Cardiac Surgery in Older Adults

Abstract: Preoperatively high neopterin levels predicted delirium after cardiac surgery in older adults, in addition to the well-known risk factors of poor cognitive function, high cardio-surgical risk, and combined CABG and valve surgery. Postoperative neopterin and HVA levels were also found to be associated with delirium, together with preoperative cognitive functioning. Plasma neopterin may be a candidate biomarker for delirium after cardiac surgery in these older adults.

“…The effect of renal function on neopterin levels has probably not influenced our results, since neopterin levels were adjusted for eGFR. Our finding of increased neopterin levels in elderly patients with a delirium is in agreement with the elevated neopterin levels that have previously been found by Osse et al [6] in patients with a postoperative delirium. …”

“…Activation of the immune system, oxidative stress, loss of neuroprotection and disturbances in cerebral neurotransmitter systems may all contribute to a delirium [6,7,8,9]. During immune activation, monocytes and macrophages are stimulated to produce neopterin in response to the proinflammatory cytokine interferon-gamma [10,11].…”

“…Además, está demostrado que en respuestas a eventos traumáticos o sistémicos, el sistema inflamatorio se activa, causando que monocitos y macrófagos produzcan neopterina, citocinas y especies reactivas de oxígeno, todos los cuales pueden ser encontrados en el (16,17,18) plasma, la orina y el líquido cefalorraquídeo de pacientes con delirium .…”

Numerous biomarkers have been correlated with delirium, 97 including inflammatory markers, 98 markers of brain injury, 99 and serum creatinine. 94 Neuroimaging findings can also correlate with delirium.…”
