Hip health at skeletal maturity: a population‐based study of young adults with cerebral palsy

Abstract: Poor hip morphology at skeletal maturity was associated with high levels of pain. Limited hip surveillance and access to surgery, rather than GMFCS, was associated with poor hip morphology. The majority of young adults who had access to hip surveillance, and preventive and reconstructive surgery, had satisfactory hip morphology at skeletal maturity and less pain.

“…Studies evaluating the immediate and long term benefits, non-benefits or harms of using standardized assessments with children and families are sparse [8,21 66,67]. Routine surveillance programmes (incorporating standardised assessments) for children with cerebral palsy have demonstrated improved bio-mechanical and pain outcomes [49,68]. Individualized client-centred tools such as DISABKIDS Chronic Generic Measure and Canadian Occupational Performance Measure have been associated with increased motivation with activity-focused interventions and improved therapy practice [63,61].…”

“…Very long‐term multicentre studies are underway, confirming the safety and durability of this approach. However, these results are only obtained in centres where children have access to three‐dimensional gait analysis, an appropriate preparation during the non‐operative phase of treatment by specialists in paediatric rehabilitation and when the surgical team has both experience and an appropriate surgical volume of at least 5 and preferably >10 children per annum …”

“…Detection of hip displacement through routine ‘hip surveillance’ is effective when it leads to earlier orthopaedic referral and earlier intervention . When surgical intervention is offered at the right time, invasive salvage procedures for hip dislocation can be avoided …”
