Among 586 children with asthma, 484 (82%) were found to have IgE-mediated ("extrinsic") asthma, and seventy-two (12%) non-IgE ("intrinsic") asthma. The remaining 30 patients (6%) were classified as "intermediate", as they had serum IgE within or above serum IgE levels of healthy children but no allergy to common allergens. During a three-year study period, the seventy-two patients with intrinsic asthma as opposed to 84 patients with extrinsic asthma had significantly more hyperinflation of the lungs, more episodes of acute hospital admissions due to asthma and/or pneumonia, more elevated serum IgG and IgM, and more cultures from secretions of lower airways of Haemophilus influenzae and pneumococci. Further, although treated with corticosteroids, eleven of the children with intrinsic asthma showed progressive disease, judged from fixed and/or declining forced vital capacity followed by signs of lung fibrosis on repeated pulmonary X-rays. It is emphasized that children with intrinsic asthma may represent an entity of childhood asthma, in some cases with severe progression of disease within a few years.

Formation of insulin fibrils is a physical process by which partially unfolded insulin molecules interact with each other to form linear aggregates. Shielding of hydrophobic domains is the main driving force for this process, but formation of intermolecular beta-sheet may further stabilize the fibrillar structure. Conformational displacement of the B-chain C-terminal with exposure of nonpolar, aliphatic core residues, including A2, A3, B11, and B15, plays a crucial role in the fibrillation process. Recent crystal analyses and molecular modeling studies have suggested that when insulin fibrillates this exposed domain interacts with a hydrophobic surface domain formed by the aliphatic residues A13, B6, B14, B17, and B18, normally buried when three insulin dimers form a hexamer. In rabbit immunization experiments, insulin fibrils did not elicit an increased immune response with respect to formation of IgG insulin antibodies when compared with native insulin. In contrast, the IgE response increased with increasing content of insulin in fibrillar form. Strategies and practical approaches to prevent insulin from forming fibrils are reviewed. Stabilization of the insulin hexameric structure and blockage of hydrophobic interfaces by addition of surfactants are the most effective means of counteracting insulin fibrillation.

To improve the in vitro diagnosis of mould allergy 22 children suffering from allergic asthma caused by Alternaria tenius and/or Cladosporium herbarum as proven by bronchial provocation test were investigated. Partially purified, standardized mould preparations were used in radioallergosorbent test (RAST) with conventional and new update mould discs, mould-induced histamine release and immunoblotting. Updated RAST discs were found to be superior to the old-type discs for the detection of Alternaria but not Cladosporium sensitivity. In all patients except one, specific IgE-antibodies to the respective mould were demonstrated by immunoblotting. Mould-induced histamine release failed to prove sensitization in only two patients. No differences were found comparing histamine release from whole blood with release from isolated cells. The results demonstrate a high sensitivity of in vitro tests when purified and standardized extracts are used.
