HIV-associated Follicular Bronchiolitis

Abstract: A 38-year-old African American, HIV-positive man (CD4 count, 347 cells/ml; viral load, 633,000 copies/ml), having never received highly active antiretroviral therapy, presented with several months of nonproductive cough. He had no other past medical history. Computed tomography of the chest ( Figure 1) demonstrated innumerable miliary nodules. Bronchoalveolar lavage (BAL) was performed and contained 71% neutrophils, 13% lymphocytes, and 12% alveolar macrophages. Flow cytometry was not performed. Bacterial, aci… Show more

“…Follicular bronchiolitis observed in HIV disease has been shown to improve with initiation of anti-retroviral drugs. 7 On the other hand, when we look at bronchiolitis patterns reported in PCD, follicular bronchiolitis has not been reported. The first case of diffuse panbronchiolitis (DPB) occurring in association with PCD in Japan was reported in 1969.…”

“…There was a satisfactory clinical and radiological response on cART 20. Finally, Shipe et al 18 presented a 38-year-old African American HIV-infected man (CD4+ count: 347 cells/µL, VL: 633 000 copies/mL and never on cART) who presented with several months of non-productive cough and radiology featuring miliary nodules. He was thoroughly examined, and there were no signs of TB, disseminated fungal disease or viral or bacterial pneumonia.…”

“…Treatment strategies include management of the underlying or associated disease, such as immunosuppressive therapy for autoimmune disease. Initiating antiretroviral therapy in a patient with HIV-associated FB has shown improvement in respiratory symptoms [11]. In patients with CVID, intravenous immunoglobulin replacement therapy has been used, as well as immunotherapy with rituximab and azathioprine to good result [12,13].…”
