Background: In 2005, the Italian Society for Artificial Nutrition and Metabolism carried out a national survey on the prevalence of home artificial nutrition (HAN), enteral (HEN) and parenteral (HPN). A second survey was carried out in 2012 to investigate the development of HAN in Italy. Methods: The Local Health Care Units (LHCUs) were required to record the ongoing cases of HAN using a structured questionnaire. The point prevalence (n./million inhabitants) was analysed according to patient age and disease categories. Data were compared with those obtained in 2005.Results: Data were obtained from 65% of the LHCUs, representing 73% of the Italian population. The prevalence of HAN was 325.5 (90.3% adults, 84.6% HEN). As in 2005, a wide range of prevalence was observed among the administrative regions. The most frequent disease category for HPN was oncological in adults and intestinal failure in children; for HEN, it was neurological in both age groups. A positive correlation was observed between the regional prevalence of HAN and the regional population aging index (p = 0.018). The prevalence of HAN in 2012 was 66% higher than that in 2005. In the individual regions, a negative association was observed between the 2012% prevalence increase of HAN (2012HAN ( :2005 and the HAN prevalence in 2005 (p = 0.020). Conclusions: A two-thirds increase in HAN prevalence was observed over seven years, probably due to improved HAN organisation and management in those regions which showed a low prevalence in 2005. Population aging seemed to be one of the main factors necessitating HAN.

Results: In all, 140 (88%) were cancer and 19 (12%) noncancer patients. Main indications were carcinosis in 68 for total, and hypophagia/dysphagia in 62 patients for partial/integrative (to oral-enteral nutrition) HPN; mean duration of HPN was 81.457110.86 days of treatment and infection rate 2.89% in the whole population and 2.66% in the 36 patients treated for more than 3 months. No other major complications have been observed. Conclusion: HPN is confirmed to be a safe and effective treatment when prescribed and administered by a trained team.

Management of Home Parenteral Nutrition: A Prospective Multicenter Observational Study

BACKGROUND: There are no specific Swiss home parenteral nutrition (HPN) data showing patient characteristics, quality of life (QoL) and complications. The goal of this study was to collect representative nationwide data on current adult HPN patients in Switzerland for international comparability and benchmarking. METHODS: This was a multicenter, nationwide, observational study. We conducted interviews for demographics, PN characteristics, QoL and complications. The data were assessed at baseline and after a follow-up of 3 months using a questionnaire. RESULTS: Thirty-three adult patients were included. The most common underlying diseases were cancer, radiation enteritis and state after bariatric surgery, and the most prevalent indication was short bowel syndrome. During the 3-month observation period, significant increase or stabilization of body weight occurred in the patients, physical activity scores improved from 34.0 to 39.4 and mental scores improved from 41.9 to 46.4. HPN dependency and traveling restrictions were of the greatest concern. Diarrhea, xerostomia and/or thirst were frequent complaints. CONCLUSION: Anthropometric parameters and QoL improved during the observational period in this HPN cohort. These Swiss HPN data are prerequisite for evaluation and comparison of HPN recommendations and best clinical practice, status of professional care instructions related to HPN effectiveness, quality of treatment and patient safety.
