Purpose The purpose of the current study was to assess the clinical and radiological results after locking T-plate osteosynthesis with coracoclavicular augmentation of unstable and displaced distal clavicle fractures (Neer type 2). Methods Thirty patients, treated between January 2007 and January 2010 were followed up after a median followup time of 12.2 months (range 4.7-37.2). The Constant and DASH scores were used to evaluate the clinical outcome, and anterior-posterior and 30°cephalic view radiographs were performed to assess the bony healing. Results In all patients, the fracture healing was achieved within the first 10 weeks after surgery. All patients regained good or excellent shoulder function and returned to previous occupation and activity levels. The mean Constant and DASH scores were 92.3 points and 6.2 points, respectively. We did not observe any severe intra-or post-operative complication within the time of followup. ConclusionThe presented technique turned out to be a reliable method providing good results without showing severe complications. Level of evidence Case series, Level IV.

ÖzetAmaç: Çalışmamızda distal klavikula kırıklarının cerrahi tedavisinde periakromiyal intramedüller K-teli fiksasyonunun omuz eklem hareketleri üzerine etkisini inceledik. Bulgular:Hastaların hepsinde kaynama elde edildi. Kaynama süresi 47.19±9.35 gündü. Hastaların %12.9'unda (n=4) pin dibi enfeksiyonu, %6.5'inde (n=2) keloid, %6.5'inde (n=2) pin migrasyonu oluştu. Ameliyat sonrası majör komplikasyon görülmedi. Yaş ile Constant skoru arasında ters yönde (Constant skoru arttığında, yaş düşmekte) %82.6 ilişki saptandı (r=-0.826; p=0.001), istatistiksel olarak anlamlı düzeyde farklılık saptandı. Cinsiyetlere göre Constant skorları arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0.05). Ayrıca komplikasyonlara göre yaş ortalamaları ve cinsiyet dağılım-ları arasında anlamlı farklılık yoktu (p>0.05).Sonuç: K-teli fiksasyonu minimal invaziv bir yöntemdir. Diğer avantajıysa K-telleri anestezi ve ek bir girişim gerektirmeksizin poliklinik şartlarında alınabilir. Distal klavikula kırıklarında peri-akromiyal K-teliyle tespit, yüksek kaynama oranı, iyi fonksiyonel sonuçlar ve kabul edilebilir komplikasyonlarıyla güvenilir bir yöntemdir.Anahtar sözcükler: Distal klavikula kırığı; eklem hareket açıklı-ğı; Kirschner teli; omuz eklemi. Background: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. Methods: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. Results: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. Conclusions: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications. (Clin Shoulder Elbow 2015;18(1):21-27)
