Hopelessness has been identified as a major psychological problem that exacerbates the clinical outcomes of chronic kidney disease (CKD) patients undergoing hemodialysis (HD). Hopelessness is mostly reduced by cognitive therapy, but the results are delayed, so a spiritual element is needed. Spiritual emotional freedom technique (SEFT) therapy can reduce hopelessness because it not only focuses on cognition, but also involves spiritual, psychological, and physical elements. This study aimed to determine the effects of SEFT therapy on the hopelessness of CKD patients undergoing HD. This research was quasi-experimental with pre- and post-test control group designs. The sample size was 64 respondents, who were divided into two groups, each consisting of 32 respondents who were selected by purposive sampling. SEFT therapy was conducted four times. Hopelessness was measured using the Beck Hopelessness Scale (BHS). The data were analyzed by paired t-test and independent samples t-test. Before receiving SEFT, the two groups did not indicate differences in hopelessness (p = 0.141). However, after receiving SEFT, the respondents’ hopelessness in the experimental group was reduced significantly (p = 0.000). Hopelessness in the experimental and control groups also differed significantly after the intervention (p = 0.000). This study revealed that SEFT reduced the hopelessness of CKD patients undergoing HD. SEFT therapy can be implemented in clinical practice areas of nursing to support patient care. Keywords: chronic kidney disease, hemodialysis, hopelessness, spiritual emotional freedom technique Abstrak Mengurangi Keputusasaan Melalui Terapi Spiritual Emotional Freedom Techniques Pada Pasien Penyakit Ginjal Kronik yang Menjalani Hemodialisis. Keputusasaan telah diidentifikasi sebagai salah satu masalah psikologis utama yang memperburuk hasil klinis pasien penyakit ginjal kronik (PGK) yang menjalani hemodialisis (HD). Keputusasaan sebagian besar dikurangi dengan melibatkan unsur kognitif, akan tetapi hasilnya mengalami keterlambatan sehingga dibutuhkan unsur spiritual. Terapi spiritual emotional freedom technique (SEFT) dapat mengurangi keputusasaan karena tidak hanya berfokus pada kognitif tetapi juga melibatkan unsur spiritual, psikis, dan fisik. Penelitian ini bertujuan untuk mengetahui pengaruh terapi SEFT terhadap keputusasaan pasien PGK yang menjalani HD. Penelitian ini merupakan quasi-experiment dengan rancangan pretest dan posttest control group. Besar sampel sebanyak 64 responden yang dibagi menjadi dua kelompok, dan masing-masing kelompok terdiri dari 32 responden dipilih secara purposive sampling.Terapi SEFT dilakukan sebanyak empat kali. Keputusasaan diukur menggunakan Beck Hopelessness Scale (BHS). Data dianalisis dengan uji paired t-test dan uji independent samples t-test. Sebelum menerima SEFT kedua kelompok tidak menunjukkan perbedaan keputusasaan (p = 0,141). Namun, setelah menerima SEFT, keputusasaan responden di kelompok eksperimen berkurang secara signifikan (p = 0,000). Keputusasaan pada kelompok eksperimen dan kontrol juga berbeda secara signifikan setelah intervensi dilakukan (p = 0,000). Studi ini mengungkapkan bahwa SEFT mengurangi keputusasaan pasien PGK yang menjalani HD. Terapi SEFT dapat diimplementasikan dibidang praktik klinis keperawatan untuk mendukung perawatan pasien. Hope therapy: Can it treat hopelessness and internal locus of control on diabetes mellitus patients?

Patients with diabetes mellitus (DM) are always required to be able to control a healthy lifestyle throughout their life to avoid various diseases that can accompany the previous illness. However, psychological factors in the form of despair due to lack of hope make people with diabetes more depressed and less able to control behavior and maintain blood sugar stability, so an excellent internal locus of control is needed to be stronger. This study aimed to determine the effect of hope therapy in reducing hopelessness and increasing internal locus of control in people with DM. The research design used a experimental study with ten randomly selected respondents divided into two groups, namely the control group and the experimental group. Data retrieval using the locus of control scale and the beck hopelessness scale. Data analysis used non-parametric analysis, namely the Mann Whitney test, Wilcoxon test, and Spearman’s Rank Correlation test. The results of the Mann-Whitney U test on the internal locus of control variable show a value of 0.000 and a p score of 0.008 (p <0.05), it can be concluded that there are differences in the internal locus of control in the experimental group compared to the internal locus of control in the control group. The hopelessness variable shows a value of 0.000 and a p score of 0.008 (p <0.05), this indicates that there is a difference in hopelessness in the experimental group and the control group. There is a decrease in hopelessness and an increase in internal locus of control in people with DM given hope therapy.

Teori hopelessness dari Beck telah dibuktikan memiliki struktur yang berbeda di populasi non-klinis. Hal ini diasumsikan karena konsep hopelessness tidak terlalu kuat pada populasi non-klinis. Penelitian pada populasi non-klinis dibutuhkan agar kita dapat memahami konstruk ini lebih lanjut. Penelitian ini bertujuan untuk meninjau properti psikometri dari Beck Hopelessness Scale (BHS) pada populasi non-klinis di Indonesia. Metode sampling yang digunakan adalah convenience sampling (N = 295) dan instrumen yang digunakan adalah translasi BHS dengan skala likert 6 skala. Properti psikometri ditinjau melalui analisis reliabilitas, daya diskriminasi, dan validitas. Reliabilitas internal consistency menunjukkan bahwa alat ukur ini memiliki reliabilitas yang tinggi (α = 0.918) dengan daya diskriminasi item berada di rentang 0.39 – 0.78. Confirmatory factor analysis menunjukkan bahwa model hierarchical (second-order) 3 faktor fit, dan mengonfirmasi teori hopelessness dari Beck (3 faktor 18 item). 2 item dari dimensi future expectations dieliminasi karena berkontribusi terhadap dimensi lain. Hasil penelitian ini mendukung hipotesis bahwa alat ukur ini memiliki properti psikometri yang baik dan dapat digunakan untuk mengukur hopelessness pada populasi umum (non-klinis) di Indonesia.Previous researches have proved that the structure of Beck’s theory of hopelessness in non-clinical populations is different from clinical populations. The underlying assumption is that the concept of hopelessness is not as well established in non-clinical populations. Research in non-clinical populations is needed for further understanding about this construct, thus the aim of this study was to explore the psychometric properties of the Beck Hopelessness Scale (BHS) in Indonesian non-clinical sample. The samples were collected using convenience sampling (N = 295) and the instrument used was the translation of BHS using a 6-point Likert-scale. The psychometric properties were examined through reliability, item discriminant, and validity analyses. Internal consistency reliability exhibited satisfactory reliability (α = 0.918) and item discriminant analysis through corrected total-item correlation ranged from 0.39 – 0.78. Confirmatory factor analysis showed that hierarchical (second-order) 3-factors model fit the data, thus confirming Beck’s theory of hopelessness (3 factors 18 items). 2 items from future expectations factor were eliminated because of cross-loadings. The findings support our hypothesis that this instrument has a good psychometric property and thus can be used to measure hopelessness in non-clinical populations in Indonesia.
