Chronic Obstructive Pulmonary Disease (COPD) is a lung disease which has a chronic obstruction in airway that can interefere normal respiration led to death. Patients with COPD usually show symptoms such as dyspnea that are most frustrating in patients with COPD that can trigger the appearance of tightness and decreased saturation. The semi-fowler position is a position that can be chosen by patients who have difficulty breathing and in COPD patients is expected to optimize lung development so that oxygen saturation becomes increased. Given the many factors that contribute to oxygen saturation in patients, it is important to control the factors that affect oxygen saturation in patients who get a semi-fowler position. In addition, given the high prevalence of COPD, it is important for hospitals to identify factors that affect the length of day of treatment of patients with COPD. Identification of these factors can help health workers to plan treatment programs to plan the length of the day of treatment of patients who enter the hospital. The study aimed to identify factors that influence oxygen saturation and the length of days of treatment in COPD patients who get semi-fowler positions at Hermina Ciputat Hospital. Data collection was carried out from January 2020 to October 2021 using the cross-sectional method, namely by taking data at once at the same time through medical record data on 84 inpatients. The analysis test used in this study is the T-test and subsequently multivariate analysis is carried out using linear regression tests. Based on the analysis test, the factors that affect oxygen saturation are gender, smoking history, and initial saturation value. While the factors that affect the length of treatment are the history of smokers and the history of comorbid. Respondents who had a history of smoking would experience a decrease in oxygen saturation by 0.3888. Preventive interventions through smoking-related health education need to be done to reduce the severity in patients with COPD who are hospitalized.

Pendahuluan: Pursed-lips breathing (PLB) adalah Latihan pernapasan yang dilakukan pada pasien dengan penyakit paru obstruktif kronik (PPOK) untuk meredakan gejala dispnea sehingga nilai saturasi oksigen pasien meningkat, dan praktiknya diajarkan secara luas sebagai strategi pernapasan untuk meningkatkan toleransi dalam melakukan aktifitas. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh PLB terhadap peningkatan saturasi oksigen pada pasien penyakit paru obstruktif kronik (PPOK). Metode: Penelitian ini menggunakan metode studi literatur review. Hasil: Hasil pencarian awal menemukan artikel potensial sebanyak 4011 artikel. Setelah membuang artikel yang teridentifikasi duplikasi dan relavan dengan topik terdapat 6 artikel yang memenuhi tema studi literatur, ke enam jurnal membahas tentang efektifitas PLB pada pasien PPOK. Kesimpulan: PLB mampu untuk meningkatkan nilai saturasi oksigen pasien, sehingga intervensi PLB dianjurkan dilakukan pada pasien PPOK dengan gejala dispnea

Early morning hyperglycaemia in diabetics can be caused by the dawn phenomenon, the Somogyi effect, or poor glycaemic control. The dawn phenomenon occurs when endogenous insulin secretion decreases or when the effect of exogenous insulin given to the patient the day before disappears, along with a physiological increase in insulin antagonist hormone. When people with diabetes eat too late at night, consume alcohol, or use insulin too late, it will cause a spike in blood sugar levels in the morning. This condition is known as the Somogyi effect and is caused by the human activity itself. The Somogyi effect is present in the case of excessive amounts of exogenous insulin. The dawn phenomenon is more common than the Somogyi effect. This research method uses a case study to explore the problem of nursing care in patients with diabetes mellitus at St Paul University Hospital, Philippines. Data were collected using interviews, observation, physical examination, and documentation. The data obtained were analysed by descriptive method, compiled from diagnosis to evaluation using three diabetic patients as sampling. To diagnose this dawn phenomenon, it is important for nurses to measure plasma glucose levels over several nights between 3 am and 5 am or use a continuous glucose monitoring system. Although the treatments are different, the best way to prevent the dawn phenomenon and the Somogyi effect is to control diabetes optimally with insulin therapy. It is hoped that nurses, clients, and families can monitor the patient's glucose levels periodically to prevent complications in diabetes.
