Hypertrophic Cardiomyopathy

Abstract: A patient with hypertrophic cardiomyopathy, syncope, and frequent ventricular tachycardia was monitored during a syncopal episode. An unrecordable blood pressure and the loss of a left ventricular outflow tract murmur without evidence of arrhythmia suggested a primary haemodynamic mechanism such as reduction in left ventricular volume.

“…Although symptoms of congestive heart failure are common and often attributed to “diastolic failure”[16], development of LV systolic impairment, and dilatation (“end stage” or “burnt‐out” disease) is less frequently observed, eventually complicating approximately 10% of cases [17]. End stage disease is associated with mortality/transplant rates as high as 10% per year [17], and indications for pharmacological and device therapy similar to those developed for systolic heart failure are assumed.…”

“…The many physiological abnormalities described in HCM often correlate poorly with morphology and symptom status [16]. Arrhythmia risk, abnormal myocardial relaxation/compliance, and ischemia can be demonstrated in patients with severe/mild LVH, unobstructed/obstructed, high/low risk, and in symptomatic/asymptomatic patients.…”
