Arcanobacterium Pyogenes and Encrusted Pyelitis

Abstract: Aim: Trueperella pyogenes is known to affect cattle, but was never isolated as a cause of human urinary tract infections. Clinical case: A 69-year-old male presented for recurring low urinary tract symptoms after a 20-day ciprofloxacin regimen for prostatitis. He previously underwent open right nephrolithotomy and left ureterovesical junction reimplantation for an iatrogenic distal ureteral stricture. Computed tomography showed spontaneous cortical calcifications; renoscopy was performed and deep cultures from… Show more

“…Even without identification of a definitive species, empirical glycopeptide use is allowed because C. urealyticum is resistant to most of antibiotics but usually sensitive to glycopeptide 3 . EC or EP is rarely caused by other urea‐splitting bacteria, such as Proteus mirabilis , Corynebacterium glucuronolyticum or Arcanobacterium pyogenes 11‐13 . In the case of hydronephrosis, drainage via a nephrostomy tube or ureteral stent should also be considered.…”

“…It is distinguished from the resembling Corynebacterium diphtheriae and Corynebacterium jeikeium due to its inability to reduce nitrates into nitrites, its asaccharolytic characteristic and its urease activity, which is highlighted by the name “urealyticum.” Although only first described as the cause of EC in 1985 ( 6 ), CU is now universally recognized as the principal cause of EC and EP. Other urease-producing bacteria reported as causative agents of encrusted uropathy are Ureaplasma urealyticum , some Streptococcus (haemolyticus and viridans ) and Staphylococcus species, Pseudomonas aeruginosa, Proteus, Escherichia coli, Corynebacterium glucuronolyticum , and Arcanobacterium pyogenes ( 7 , 12 14 ).…”
