ABSTRACTobserved after laminectomy and revealed by magnetic resonance imaging (MRI) (22). Recently, more attention has been focused on PF, which may be responsible for recurrent pain after laminectomy (9,16) to prevent post laminectomy scarring and to induce new bone formation in different approaches (5,8,30). A number of synthetic and natural materials have been used to prevent or reduce PF and induce new bone formation after laminectomy in both animal and human studies (22). Especially, the PASS principles (Primary wound closure/ █ INTRODUCTION P eridural fibrosis (PF) is an undesirable outcome of decompressive spinal surgery that may compress the dura and nerve roots (3). PF develops out of the laminectomy membrane, which covers the defect created by the bone resection to replace the resected areas. Commonly, the bone defects as a result of laminectomy regenerate as fibrous tissue that intends to regenerate resected lamina (24). Briefly; PF is a part of the natural healing process and it could be AIm: A quantitative model of postlaminectomy was designed in rats. The effects of Momordica Charantia (MC) and Ankaferd blood stopper (ABS) on the bone and scar formation after laminectomy were concurrently evaluated. mATERIAl and mEThODS: Eighteen adult Wistar albino rats underwent lumbar laminectomy at L2-L3 vertebral levels, and were randomly assigned to one of three groups of six rats each. The Treatment group received MC and ABS treatment and the Control group was left untreated. Rats were sacrificed 4 weeks after treatment. Then; the lumbar spine was excised en-block, fixed and decalcified. Sections were stained with hematoxylin and eosin (H&E) and Masson's trichrome, and evaluated for peridural fibrosis (PF), new bone formation, and vascular proliferation. RESUlTS:Total volume of new bone in the MC group was significantly increased in comparison to the Control group (p<0.05). Also; there was highly significant increase in terms of the total volume of fibrous tissue in the MC and ABS groups when compared with the Control group (p<0.01). Besides; there was a highly significant difference between the MC and the Control groups (p<0.01) in point of total volume of vessel.CONClUSION: Both MC and ABS are not convenient to prevent the PF formation and MC may promote new bone formation and angiogenesis after lumbar laminectomy in rats.

Diabetic foot ulcers (DFUs) are very important diabetes-related lesions that can lead to serious physical consequences like amputations of limbs and equally severe social, psychological, and economic outcomes. It is reported that up to 25% of patients with diabetes develop a DFU in their lifetime, and more than half of them become infected. Therefore, it is essential to manage infection and ulcer recovery to prevent negatives outcomes. The available information plays a significant role in keeping both physicians and patients aware of the emerging therapies against DFUs. The purpose of this review is to compile the currently available approaches in the managing and treatment of DFUs, including molecular and regenerative medicine, antimicrobial and energy-based therapies, and the use of plant extracts, antimicrobial peptides, growth factors, ozone, devices, and nano-medicine, to offer an overview of the assessment of this condition.2 of 32 history [2]. DFUs are one of the most severe complications of diabetes, and more than half of those ulcers become infected. Every single one of these infected lesions has the potential to get worse and compromise the integrity of the lower limbs. To avoid amputations and improve the patient's quality of life, it is very important to implement a strict program for the prevention and treatment of ulcers, as well as proper management of infections [3]. Thus, it is critical to keep diabetes patients aware of new therapies and treatments and their availability in the healthcare system. The treatment of DFUs requires a multidisciplinary approach with proper medical tools, skills, and knowledge. This starts from patient education, with the application of new classifications to guide the treatment to prevent amputations. New diagnosis methods should become available, such as the 16S ribosomal DNA sequence in bacteria, to provide a better understanding of the microbiota in DFUs. It is reported that DFU has a polymicrobial nature, and, according to its geographical location, certain marked differences, wound characteristics, antibiograms according to local epidemiology, individualized antimicrobial guided therapy, regular debridement, regular assessment of wounds, and change of dressings. The latter characteristics are also aided by new biological and molecular therapies that were proven to improve infection control, the regulation of the local inflammatory profile, and improved quality of the cicatrizing process. Diabetic foot wounds are a major cause of morbidity and mortality 1 and, even in centers in which nearly half the patients with moderate or severe wounds undergo revascularization, these wounds have median healing times of 6 months or more, with one-third requiring amputation. 2Such resource-intensive care is simply not available in most of the world, where "water or even gauze dressings may be scarce or not available".1 Therefore, topical formulations to assist in wound healing are of significant interest: they can be simply applied, are not invasive, do not require advanced training for application, and can be used in an outpatient setting at relatively low cost. Moreover, many wounded patients have gone through exhaustive treatment with offloading, daily wound care, and invasive procedures, all without successful healing. 3Topical agents for wound healing have been described and used since antiquity.The study reported in the current issue of the Journal of Diabetes 4 is therefore of interest in exemplifying the importance of research to find novel approaches to effective topical agents. Opiate receptors are present in the skin and stimulate keratinocyte formation.5 Gupta et al. 4 used a delayed excisional wound-healing model in Zucker diabetic fatty mice to compare topical fentanyl with a saline-containing cream, finding significantly greater wound closure at 18 and 28 days, with a reduction in wound edema and increased re-epithelialization, vascularization, and granulation tissue formation with greater levels of collagen deposition. There is emerging data for the use of a number of topical antimicrobial, debriding, angiogenic, and cellular therapies as agents for wound healing.6 Probiotics and nitroglycerin have been proposed as offering therapeutic potential. 7,8 Oxygen has been studied using a variety of application methods.9 Natural substances that have been studied include preparations based on beehive protectant propolis, 10 aloe vera, 11 Momordica charantia fruit, 12 and kiwifruit.13 A recent meta-analysis suggested benefit of platelet-derived growth factor, 14 and both epidermal growth factor 15 and macrophage-stimulating agent 16 have been studied as additional biological approaches.Although there is much interest in these topical treatments, none appears yet sufficiently well validated to be ready for widespread use. Topical honey for promotion of wound healing was studied in a Cochrane Database Systematic Review, which reported no benefit of such an approach. 17
