IL-5 Predominant in Bronchoalveolar Lavage Fluid and Peripheral Blood in a Patient with Acute Eosinophilic Pneumonia.

Abstract: Wedescribe an acute eosinophilic pneumonia (AEP) patient with bronchoalveolar lavage fluid (BALF).Eosinophil cell number (47%), content of interleukin (IL)-5 (8.22xlO2 pg/ml) and eosinophil cationic protein (9.25 jig/ml) were high in BALF. No eosinophilia was seen in peripheral blood on admission; however, content ofIL-5 was 9.47xlO2 pg/ml. After methylprednisolone pulse therapy, he improved rapidly with a reduction in eosinophil cell number (7%) and the content of IL-5 (<100 pg/ml) in BALF. However, a high co… Show more

“…The number of reports describing AEP cases has recently increased and several of these have demonstrated the elevation of IL-5 levels in serum and BAL fluid [4][5][6]. Some reports have described the role of IL-5 in the pathogenesis or disease activity of AEP [4,17].…”

“…An adequate history with respect to cigarette smoking was available in 28 cases; 10 patients were nonsmokers and 18 were smokers. Of the 18 occurrences in smokers, 9 met our criteria for AEPfollowing cigarette smoking ( Table 1); 8 of these 9 reports originated from Japan (20)(21)(22)(23)(24)(25). The ninth case was reported by Godding and colleagues who described AEP in a 14-year-old girl who recently had begun smoking cigarettes, although the authors did not identify smoking as a precipitating cause of AEPin their report (26).…”

“…An array of both Th2 and Th1 cytokines are elevated in the BAL fluid in AEP, including IL-1ra, IL-2, IL-5, IL-10, IL-12, IL-13, and IL-18, perhaps reflecting the known pluripotent secretory potential of eosinophils (3,76,180). In particular, elevated BAL fluid levels of IL-5, the key cytokine promoting differentiation, migration, and survival of eosinophils, are associated with AEP (120,134,155,184,215). Cationic granule proteins and inflammatory lipid mediators are detectable in the urine of subjects with AEP (136).…”
