Imaging features of renal complications after crizotinib treatment for non–small-cell lung cancer: a case report

Abstract: Crizotinib has been approved for the treatment of advanced ALK-positive non–small cell lung cancer. Its use is associated with the development of complex renal cysts. However, there is limited literature regarding imaging features of renal cystic disease during crizotinib therapy and its complications or progression. Here, we describe a case of a patient with ALK-positive advanced non–small cell lung cancer who developed complex renal cyst during crizotinib treatment. The renal cyst is complicated by infection… Show more

“…9 It has been suggested only a close laboratory and radiological monitoring are necessary in asymptomatic cysts; although, in rare cases, crizotinib dose reduction or discontinuation is mandatory. 9,12 Here we report the onset of complex renal cysts, perirenal cysts that invaded adjacent structures such as the iliopsoas muscle, and two hepatic cysts during the crizotinib treatment. The local extension of the cysts to adjacent tissues would require percutaneous drainage if the mass effect became clinically relevant.…”

“…6,14,15 A superinfection of crizotinib-induced cysts was reported, so blood cultures were performed to exclude this condition. 11,12 The hepatic cysts had the same radiological aspect as the renal ones, so biopsy had not been deemed necessary to exclude their metastatic nature.…”

“…Another case report study showed an individual with anaplastic lymphoma kinase-positive advanced non-small cell pulmonary carcinoma who detected complex kidney cyst during crizotinib therapy. (51). Epidermal growth factor receptor can be bound by ligands that regulate renal inflammation, cell growth, and fibrosis, potentially leading to kidney damage, as summarized by Rayego-Mateos et al (52).…”

“…At present, targeted therapy has become one of the most important treatment methods in NSCLC, especially in lung adenocarcinoma with the mutation of driver genes, including c-ros oncogene 1 (ROS1), whose mutation is a rare molecular driver of NSCLC that can be effectively treated with crizotinib ( 3 ). However, crizotinib has been found to have an adverse effect on the development of renal cysts ( 4 , 5 ), with an incidence of 5% showing in the drug instructions. In Asian patients, the incidence of progression of existing renal cysts and the development of new renal cysts are higher in patients who were treated with crizotinib ( 4 ).…”
