Imaging of a Case of Extramedullary Solitary Plasmacytoma of the Trachea

Abstract: We describe a case of extramedullary tracheal plasmacytoma that was incidentally discovered in a 73-year-old man on a PET scan performed for assessing the extent of colon cancer. CT scan showed the tumor; multiplanar reformation coupled with virtual bronchoscopy allowed proper treatment planning. The tracheal tumor was resected during rigid bronchoscopy. Relevant investigations excluded multiple myeloma. Follow-up CT showed persistent thickening of the tracheal wall, but there has been no recurrence after one-… Show more

“…[ 6 ] Squamous cell carcinoma and adenoid cystic carcinoma are the malignant tumors most common affecting the trachea. [ 7 ] EMP of the trachea, primarily occurring in 50-to-60-year-old male patients, represents around 3% of malignant tracheal tumors. [ 7 , 8 ] The pathogenesis of tracheal EMP is still unclear, but viral pathogenesis and chronic irritation may contribute to those lesions.…”

“…They constitute around 4% of all plasma cell neoplasms and in 80% of cases occur in the upper aerodigestive tract-nasal cavity, paranasal sinuses, and oronasopharynx. [ 1 2 3 ] Trachea is a very rare site of plasmacytoma. Less than twenty cases were reported in the medical literature until 2013.…”

“…Involvement of the larynx, hypopharynx, cervical glands, esophagus, cervical lymph nodes, middle ear, and mastoid is rare (5) , and tracheal involvement is even rarer (5,11-13) , occurring in only 3% of all extramedullary plasmacytomas (9) . As of 2005, only 15 cases of solitary extramedullary plasmacytoma of the trachea had been reported in the medical literature (8) .…”

“…Bronchoscopy correlates well with CT and can be used in order to resect the lesion (5,7) . The diagnosis is made through histological and immunohistochemical studies (5,14) . There was one reported case in which the tracheal plasmacytoma was identified as an incidental finding on positron emission tomography/CT (5) .…”
