The question whether an implant could be placed in an infected site has often been raised and addressed in the literature by several authors. There are several studies that have successfully placed implants in infected sites [1][2][3][4][5][6]; however, all these studies removed granulation tissue before placing the implant as a part of their clinical protocol [7][8][9][10][11][12][13][14][15][16]. In this study, the implants were placed in infected sites without curettage.A prospective clinical study was done with a total of 10 cases. The selected infected, maxillary anterior teeth presented with either chronic or acute periapical lesions with poor to a hopeless prognosis. A standardized surgical and prosthetic protocol was developed. Implants were placed and cases were evaluated at six-month follow-up. Using pre-and postoperative cone beam computed tomography (CBCT), data were collected [16][17][18][19][20][21][22]. The result reflects and confirms the hypothesis that endosteal implants can be successfully integrated in infected sites. However, alveolar ridge resorption after the tooth extraction is a frequent occurrence and may significantly reduce the residual bone volume [23][24][25][26][27][28]. This study is a proof of principle that sockets can heal naturally with immediately placed implants in infected sites and tissue shrinkage can be reduced by utilizing least invasive surgical and prosthetic protocol.

The high survival rate obtained in several studies supports the hypothesis that implants may be successfully osseointegrated when placed immediately after extraction of teeth presenting endodontic and periodontal lesions, provided that appropriate clinical procedures are performed before the implant surgical procedure such as meticulous cleaning, socket curettage/debridement, and chlorhexidine 0.12% rinse. However, more randomized controlled clinical trials with a longer follow-up are required to confirm this procedure as a safe treatment. Moreover, the outcome measures were not related to the type of infection; the classification of infection was often vague and varied among the studies. The benefits of antibiotic solution irrigation and systemic antibiotic administration in such conditions are not yet proved and remain unclear.

Dental implants placed in fresh extraction alveoli provide several advantages, including shorter treatment periods and improved patient comfort. After a compromised tooth extraction, the Er,Cr:YSGG laser can considerably reduce bacterial concentration. The objective of this controlled study conducted after at least 1 year of follow-up was to compare the use of immediate post-extraction implants in infected sites treated with laser (test group) versus conventional implants in edentulous sites (control group) through an analysis of pre- and post-operative radiographs. The study was based on a series of patients treated between 2014 and 2019, with a 1-year minimum follow-up, and up to over 4 years. An analysis of the clinical history of the treated patients and pre- and post-operative radiographs was performed to evaluate the implant success and to measure the marginal bone level (MBL). Overall, 149 implants were studied. There was only one failure in the test group (1%) and no failures in the control group. The test group gained 0.1 mm of the MBL compared to the baseline, while the control group lost 0.1 mm of the MBL. The difference between the two groups of only 0.2 mm was not statistically significant (P = 0.058). Immediate dental implants in infected sockets debrided and decontaminated using Er,Cr:YSGG laser do not appear to enhance the likelihood of failure; however, peri-implantitis and associated problems must be avoided by following a certain set of protocols and procedures.
