Immunoadsorption in Guillain‐Barré Syndrome and Myasthenia Gravis

Abstract: Elimination of circulating antibodies by hemapheresis is an empirical treatment concept in various neuroimmunological diseases. Plasma exchange (PE) has been shown to be superior to symptomatic treatment in Guillain-Barré syndrome (GBS) in two large multicenter studies. It is also effective in myasthenia gravis (MG), although no comparative studies have been performed. Immunoadsorption (IA) using polyvinyl alcohol gel columns to which phenylalanin (IM-PH) or tryptophan (IM-TR) are covalently bound is an altern… Show more

“…As a result of the anonymous design it was not possible to assess patients' exact clinical status of MG according to the MGFA Classification [22] or to check patients' reports with medical records. Due to the large sample size, clinical assessments of each MG patient in the whole country would have required intensive use of trained neurologists; such a procedure was impossible to implement.…”

“…Interventional strategies for the rapid elimination of pathogenic antibodies can be offered by therapeutic apheresis [e.g., plasma exchange (PE) or immunoadsorption (IA)] [17,18]. PE or IA are most effective in circumstances when rapid treatment effects are required, in particular in patients with progressive deterioration of symptoms of MG or myasthenic crisis, especially, in AChR-Ab positive and MuSK positive patients [2,3,10,19,[20][21][22].…”

“…Реакция связывания опреде-лённых молекул основана на реакции антиген-ан-титело. Метод иммуносорбции позволяет удалять из циркулирующей плазмы Ig без необходимости использования замещающих растворов, таких как раствор альбумина или свежезамороженной плазмы, что уменьшает риск возникновения ал-лергических или инфекционных осложнений [11,16]. Иммуносорбционные колонки содержат гель-сорбент, состоящий из триптофана, ковалентно связанного с поливиниловым спиртом.…”
