Immunocytochemical and Ultrastructural Evidence Supporting That Andes Hantavirus (ANDV) Is Transmitted Person-to-Person Through the Respiratory and/or Salivary Pathways

Abstract: Pizarro et al. Hantavirus Respiratory and Salivary Transmission Study ANDV antigens were in capillary endothelium, the secretory cells and filling the lumen of the excretory pathway. It is proposed that in patients with HPS caused by ANDV the alveolar epithelium and macrophages would be the gate for the airway spreading of the virus, while the salivary glands are a target for virus replication and an exit pathway through saliva.

“…Despite the severity of these diseases, no efficient treatment is available. Inhalation of aerosols contaminated with infected rodent excreta is the major route of transmission, although person-to-person transmission of a pulmonary syndrome caused by Andes hantavirus (ANDV) has also been reported (Martinez et al, 2005;Padula et al, 1998;Pizarro et al, 2019). NWHs therefore have the potential to adapt to humanto-human air-borne transmission routes, increasing their epidemic potential.…”

“…Other hantavirus species, such as the Sin Nombre virus (SNV) and Andes virus (ANDV), that are prevalent in North and South America, respectively, can cause human hantavirus cardiopulmonary syndrome (HCPS) 8,9 . In addition, ANDV is known to be transmitted from person to person through the respiratory tract and saliva 10 . Infection with these viruses may cause various clinical presentations, from asymptomatic to acute fever and hemorrhage, acute kidney or lung injury, and even hypotensive shock 11 .…”

“…The presence of ANDV in the alveolar epithelium and salivary glands of Sigmondontine rodents reinforces intraspecies transmission from saliva and biting (Padula et al, 2004). ANDV infected patients have shown the virus to be present in pneumocytes and pulmonary macrophages, with ultrastructural and immunocytochemical studies revealing viral replication occurring in the alveolar epithelial cells with virus-like particles being released into the alveolar lumen (Pizarro et al, 2019). ANDV is likely secreted into human saliva and transmitted through close, intimate encounters or by exposure to respiratory droplets released through coughing or sneezing.…”
