Background Enlarged intra‐parotid lymph nodes (pLNs) may be misdiagnosed at ultrasound (US) being rare entities and because of their similarities to parotid nodules. Fine‐needle cytology (FNC) is used in the preoperative diagnosis of parotid nodules, and may be used on pLNs to assess their nature and the underlying pathological processes. Methods Twenty consecutive pLNs underwent US‐guided FNC and rapid on‐site evaluation (ROSE). Flow cytometry (FC) was also performed in all the cases. Immunoglobulin heavy chain (IGH) gene rearrangement PCR was performed in five cases. Data obtained were checked by follow‐up and histological controls, when available. Results Ultrasound‐guided FNC and ROSE identified 20 pLNs. According to FNC‐FC and PCR data, pLNs were diagnosed as reactive processes(18), FL (1) and B‐cell, NHL NOS (1). Clinical follow‐up (18 cases) and histological assessment (three cases) confirmed FNC‐FC diagnoses of reactive processes and two NHL. Conclusions Fine‐needle cytology is a sensitive procedure in the identification of pLNs. FNC is useful in the management of pLNs; it allows a simple follow‐up in case of reactive processes and surgical excision in case of NHL, thus sparing useless excisions for reactive processes.

Minimally invasive procedures such as endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) must yield not only good quality and quantity of material for morphological assessment, but also an adequate sample for analysis of molecular markers to guide patients to appropriate targeted therapies. In this context, cytopathologists worldwide should be familiar with minimum requirements for refereeing cytological samples for testing. The present manuscript is a review with comprehensive description of the content of the workshop entitled Cytological preparations for molecular analysis: pre-analytical issues for EBUS TBNA, presented at the 40th European Congress of Cytopathology in Liverpool, UK. The present review emphasises the advantages and limitations of different types of cytology substrates used for molecular analysis such as archival smears, liquid-based preparations, archival cytospin preparations and FTA (Flinders Technology Associates) cards, as well as their technical requirements/features. These various types of cytological specimens can be successfully used for an extensive array of molecular studies, but the quality and quantity of extracted nucleic acids rely directly on adequate pre-analytical assessment of those samples. In this setting, cytopathologists must not only be familiar with the different types of specimens and associated technical procedures, but also correctly handle the material provided by minimally invasive procedures, ensuring that there is sufficient amount of material for a precise diagnosis and correct management of the patient through personalised care.

Background: Most cases of non-Hodgkin lymphoma (NHL) can be diagnosed using a combination of fine-needle cytology (FNC) and flow cytometry together with immunoglobulin light chain restriction and/or specific phenotypic profiles. However, 5%-15% of B-cell NHLs lack these specific diagnostic features. In such cases, the diagnosis of NHL may be supported by molecular clonality testing based on the immunoglobulin heavy chain (IGH) assay of clonality by polyacrylamide heteroduplex analysis or by automated capillary electrophoresis via GeneScan analysis. Chip-based microfluidic technology (MT), based on miniaturized parallel capillary electrophoresis structures, is a viable alternative to capillary electrophoresis analysis, being less costly and cumbersome. In this study, we evaluated the performance of MT platform in IGH clonality assessment in a series of lymph node FNC samples.Methods: Thirty-five consecutive lymph node FNCs were evaluated. In all cases, the first and the second passes were used to prepare a conventional smear and to collect material for flow cytometry analysis; residual material was collected for molecular clonality assessment, and PCR products were analyzed both by MT and GeneScan platforms.Results: Molecular clonality assessment by MT had a sensitivity of 84.2% and a specificity of 76.9%; GeneScan analysis had a sensitivity of 88.8% and a specificity of 92.8%. The overall agreement between the two platforms was 85.7% (30/35).Conclusions: MT analysis proved to be a viable technique for IGH clonality assessment on FNC samples. Should our data be confirmed in larger studies, the MT procedure may be suitable for routine diagnostic practice, even on cytological samples.
