Immunopathogenesis and management of polymorphic light eruption

Abstract: Polymorphic light eruption (PLE) is the most common immunologically mediated photodermatosis, demonstrating many abnormalities caused by critical failure of ultraviolet (UV)-induced immunosuppression. The unique expression of antimicrobial peptides in PLE, which is most likely determined by alteration of microbiome components upon UV exposure, implicates their possible triggering role and pathogenic significance in the eruption. The review aims to clarify current knowledge

“…First-line treatment, which in general is thought to all diseases in this section includes sun avoidance, broad SPF sunscreens and topical corticosteroids to reduce the inflammation. However, the user-defined low-dose sun exposure or narrowband UVB and UVA lamps had been shown to be effective, presumably due to the induction of endogenous tolerance and balanced immune response [ 227 ]. Chronic actinic dermatitis is another rather common immunologically mediated photodermatosis that affects the elderly population with high exposure to UVR [ 228 ].…”

“…In addition to sun protection and topical steroids, the patient began treatment with this extract and reported reduced duration and severity of skin eruptions in the following months. Additionally, Kadurina et al suggest afamelanotide, an α-melanocyte stimulating hormone analogue, may be a promising future therapeutic option for polymorphic light eruption (PMLE) [14]. Afamelanotide induces skin pigmentation, enhances DNA repair, and induces antioxidant defense via binding to the melanocortin-1 receptor.…”

“…Polymorphous light eruption (PLE) is considered the most frequent idiopathic photo dermatosis, representing 70% of all photo-induced skin eruptions. 1 The exact etiology and pathogenesis of this condition is currently unknown. There are different therapeutic options for PLE, the most frequently used being preventive measures such as photo protection and desensitization with UVB phototherapy.…”

“…There are different therapeutic options for PLE, the most frequently used being preventive measures such as photo protection and desensitization with UVB phototherapy. 1 In severe cases, some patients need to be treated with immunosuppressants such as azathioprine or ciclosporine. 1 Omalizumab is an anti-IgE antibody used for the treatment of IgE-mediated diseases.…”
